The aim of this study is evaluation of effectiveness of chronic disease self-management program on self-care and self-efficacy in patient after coronary artery bypass grafting And be performed for the one group, before and after. Populations in this study are patients that two month after coronary artery surgery refer to Kermanshah Imam Ali cardiac rehabilitation center. Estimates of the number of samples are 52 patients. And be chosen as available between the people who consent to the participate in this program. Stanford university chronic disease self-management program is a standard program and In relation to public issues that chronic patients are faced to them. This Program for six weeks and one class, and two and a half hour per week are carried out. The Program is based on Bandura's self-efficacy theory and emphasize on three matters: planning ability, problem solving, decision making. Before, end, and three months after the program, by sherer's general self-efficacy questionnaire and health status assessment questionnaire will be evaluated.Demographic data is collected with a researcher made form. Finally the finding will analyzed by statistical tests.
