Abstract
Purpose: the analysis of the impact of insulin glargine and acarbose compared to insulin glargine and metformin in patients with type II diabetes
Design: randomized clinical trial, double-blind, placebo-controlled loading, single-center
Procedure: 152 patients with diabetes are divided to four groups with balanced dynamic randomization where each group contains 38 individuals. The first group (group A) uses placebo metformin and placebo Acarbose. The second group (group B) takes 500-mgl metformin pills every night; the drug dose increases for 2 grams every week, and they also use placebo Acarbose. The third group (group C) use 100 mg of Acarbose pill. The dose in the first week is 50 and then 100 mg for each meal (utmost daily doze is 300 mg); they also use metformin placebo. The fourth group (group D) uses metformin and Acarbose pill. Diet and sport is prescribed for all of the groups. Their blood is analyzed and they are clinically examined. A questionnaire is also distributed among them.
Participants: Diabetes patients who refer to clinic (criteria for inclusion: type II diabetes, age range of 18-75, exclusion criteria: suffering from chronic diseases, the consumption of insulin)

Major outcome: Fasting blood glucose, blood sugar two hours after breakfast, glycosylated hemoglobin, creatinine, triglycerides, cholesterol, high density lipoprotein, alanine aminotransferase, aspartate aminotransferase
