Objectives: Endoscopic sinus surgery as the treatment of choice is widely used especially in patients with chronic sinusitis that does not respond to medical therapy. The aim of this study is to compare the effect of labetalol with dexmedetomidine in reducing blood pressure and bleeding during endoscopic sinus surgery. Design: Block randomization, double blind, without placebo, trial phase 2, including 100 patients who are candidate for endoscopic sinus surgery. Setting and conduct: Fentanyl, 1 µg/kg and propofol, 2.5 mg/kg IV administer to induce anesthesia. 0.5 mg/kg Atracurium IV infusion for muscle relaxation.Major inclusion criteria: ASA class I to II; Age between 18 to 60 years old; Patients satisfaction. Exclusion criteria: pregnancy; history of cardiovascular diseases (Arrhythmia, ischemia, and heart block); history of coagulopathies; taking opioids; History of sensitivity to the drugs used in this study. Intervention: Dexmedetomidine, 1 µg/kg as initial dose and 0.4 to 0.8 µg/kg/h IV infusion. labetalol 0.25 mg/kg as initial dose and1 to 2 mg/min IV infusion
