This study aims to study the impact of telephone follow-up care after discharge on quality of life of patients with acute coronary syndrome. this is single blind study. A total of 74 patients with acute coronary syndrome who referred to Imam Reza hospital in Kermanshah will be selected and randomly will be assigned into two control and intervention groups. intervention group will complete the quality of life questionnaire after hospital discharge, then will receive the after hospital discharge training and for 3 months after discharge they will receive telephone follow-up by the researcher, follow up will be by mobile phones (two calls a week, 8 am to 8 pm and the patient organizing and determining time). Average time of each calls is 20 minutes and the first call is three days after discharge, after the intervention, the questionnaire will given to them again. control group after completing the questionnaire, only will receive the after discharge from the hospital training and the questionnaire will given to them again, The scores in each group, at baseline and 3 months after the study, will compared.
Inclusion criteria: Age 70-35 years; patient satisfaction; diagnosis of uncomplicated acute coronary syndrome; having phone line; not having a mental illness and retardation
Exclusion criteria: Hospitalization; disconnect the phone for 2 weeks during the first month and 3 weeks during the second and third month of intervention.

Description
Intervention group will given the quality of Life questionnaire after hospital discharge, Then will receive the after hospital discharge training and for 3 months after discharge they will receive telephone follow-up by the researcher.
follow up will be by mobile phones (two calls a week, 8 am to 8 pm and the patient organizing and determining time) . average time of each calls is 20 minutes and the first call is three days after discharge, The content of conversations including (recognition of disease and symptoms of, drug use and awareness of complications, self-care, nutrition, control of vital signs, level of sexual activity, smoking cessation, weight management and blood sugar, no alcohol and alcoholic beverages consumption, stress reduction). After the intervention, the questionnaire will given to them again. The scores in each group, at baseline and 3 months after the study, will compared.
