In this research 60 adult patients that were admitted for open heart surgery for the first time, had 18 to 80 years old, had no history of mental illness and earned moderate to high scores from Beck Anxiety Questionnaire were assigned to intervention and control groups randomly. Intervention group patients received Spiritual care and Quad Communications reform educations according to Ghalbe Salim model on admission to the hospital before open heart surgery. The control group received no intervention. Anxiety level was studied in both intervention and control groups before surgery and time of hospital discharge.

Inclusion criteria: Getting the 21 points and above from Beck anxiety questionnaire; Patients that were admitted in hospital for the coronary artery bypass graft surgery (CABG) for the first time with age from 18 to 80 years; having of hearing and speech ability; no being addicted to drugs; Lack of mental illnesses.
Exclusion Criteria: Hospital discharging with personal satisfaction; no wanting to research continuing.
