This study has been conducted to investigate impact of initiating rapid response nursing team on survival of patients in emergency department and medical ward. 714 patients who had admitted to these wards in hospital were randomly allocated to two groups (study: 357, control: 357). Rapid response nursing team consisted of an intensive care nurse and an anesthesia technician attended 24 hours during the week. All necessary interventions for control group were done by the wards’ staffs according to the orders or protocols. The rapid response nursing team had been called by staffs of medical ward or emergency department whenever patients’ condition deteriorated like decrease in LOC and SaO2, dyspnea, bradycardia,…. The team used to be immediately attendant and helped the staffs to evaluate patients and intervened accurately; if the staffs were not enough expert, the team itself performed all necessary interventions based on physician orders or hospital protocols such as O2 therapy, positioning, ECG, blood drawing for laboratory tests,…. After each intervention the outcome including stabilization of patients condition, need to ICU, CCU admission or cardiopulmonary arrest and unexpected death was recorded.

Description
Intervention: Rapid response nursing team consisted of an intensive care nurse specialist and an anesthesia technician who attended in emergency department 24 hours during the week. They rounded emergency department and medical ward at the beginning of each work shift and then every 2 hours as needed. They have been called to attend for patients who experienced deterioration in clinical condition like bradycardia , tachycardia, bradypnea, tachypnea, decrease in LOC, SaO2 and BP, …..The team used to be immediately attendant and helped the staffs to evaluate patients; If the staffs were not enough expert, the team itself performed all necessary interventions according to physician orders or hospital protocols such as O2 therapy, positioning, ECG, blood drawing for laboratory tests,… After each intervention the outcomes including stabilization of patient condition, need to transfer to ICU or CCU, transfer to other centers or cardiopulmonary arrest and unexpected death was recorded.
