This Study Was Conducted With The Goal Of Determining The Effect Of Community Health Nurses’ Educational Intervention On The Self-Care Of The Patients With Type 2 Diabetes Who Referred To Hazrat Ali Clinic In Isfahan. The Inclusion Criteria Were Comprise Patients With Type 2 Diabetes, With A Mean Age Of 40–70 Years, Who Had A Medical Record In The Health Care Center And Were Under The Coverage Of Hazrat Ali Clinic And Exclusion Criteria Were Comprise Unwillingness Of Patient To Continue Cooperation In Educational Intervention. The Study Population Were All Patients With Type 2 Diabetes. Sampling Was Completed With Using The Pocock Formula And The Results Of The Pilot Study And A Total Of 50 Subjects Were Selected And Assigned To Two Groups Of Study And Control(25 In Each Group ). Educational Intervention Was Conducted In The Study Group Through Seven Educational Sessions And The Control Group Underwent An Educational Session And Was Given A Self-Care Educational Booklet. Primary Outcome Was The Mean Score Of Self Care After The Intervention.
