The effect of education after discharge and follow-up based on the educational package on self-care behavior in patients with ischemic heart disease in the Cardiac care units of Gorgan Medical and Educational Centers
The aim of this clinical trial is determine the effect of education after discharge and follow-up based on the educational package on self-care behavior in patients with ischemic heart disease in the Cardiac care units of Gorgan Medical and Educational Centers, 2015.
Population and sample: The study population include of patients with ischemic heart disease, who refer to the cardiac care units of Gorgan hospitals at the time of the study, in 2015.
90 patients will be selected and after obtaining informed consent, they will be randomly assigned into two experimental and control groups (n = 30 per group).
In the control group will be implemented routine discharge plane. The intervention consisted of two groups. In the first intervention group, patients will be educated about discharge plane routinely and will be telephone follow-up for one month. The second intervention group, patients will be receive educational package includes discharge education and educational booklet and will be follow up by telephone during one month.
At the beginning (day discharge) self-care behavior questionnaire will be completed and then at 15 and 30 days after discharge, this questionnaire will be completed for all three groups.
In the end, after analyzing data, self-care behaviors among the three groups will be compared.
