The purpose of semi- experimental study is to investigate the effect of nursing consultation on patient's family anxiety and satisfaction at the Cardiac surgical intensive care unit. Inclusion Criteria: the family member is responsible for the patient care, He is patient undergoing open heart surgery, etc. Exclusion criteria: family member Refuse to participate in the study, The patient's postoperative complications such as bleeding, loss of consciousness, death, etc. The sample consisted 47 on patient's family members at the Cardiac surgical intensive care unit in Amir almomenin, Kordkoy, Iran. The convenience sampling random selection, there were 24 people in intervention group and 23 people in control group. In the intervention group: counseling by nurse researcher with family member from the time of admission to hospital for heart surgery until discharge from the hospital three times to half an hour, Face to face, including the preparation before surgery, explanation about the disease, cardiac surgical intensive care unit, inpatient treatment program at the time, medication and care plan after discharge, Emotional support to families the opportunity to express feelings, to answer the questions. Control group: No specific intervention is not the only information the hospital routine cardiac surgery and intensive care provided to the patient's family. Primary outcomes: patient's family anxiety be measured by Spielberger questionnaire. Secondary outcomes: satisfaction with information received by a family member questionnaire
