Carpal tunnel syndrome is the most common compressive neuropathy of the upper extremity. There are a variety of treatment methods for carpal tunnel syndrome. The choice of treatment depends on the severity, the duration of symptoms, and patient preference.
Aim: evaluation of a less invasive outpatient method for treatment of carpal tunnel syndrome.
inclusion criteria: severe carpal tunnel syndrome
exclusion criteria: previous neuropathy or nerve injury
study population: 20 patient with severe carpal tunnel syndrome
intervention: ultrasound guided needle fenestrating of carpal ligament of wrist
primary outcome: clinical improvement
secondary outcome: sonographic improvement

Description
intervention: patient lies prone an wrist sets in supine position. median nerve is visualized by sonographic guidance at wrist area.after local analgesia a 22 gauge needle will be entered from ulnar side of median nerve and carpal ligament starts to be fenestrated. at the end 40 miligram of Triamcinolone is injected and needle is removed.
