Settings and conduct
This study is done at Rafidean Rehabilitation Hospital. The participants in the study are assessed using the Bradan scale and the entry score is recorded. Then, the study was conducted by the researcher based on the prevention of compression scars for 4 sessions of 1 hour in 4 days. And the rehab team is formed based on the needs of the patient and the family in their presence. After three weeks, the risk score of the compression wound is measured and the score is compared before and after the intervention.
Participants/Inclusion and exclusion criteria
Criteria for entering the study :Braden is 18 or less , varying degrees of motor restriction. Exit criteria: hospital stay less than 3 weeks
Intervention groups
After obtaining the consent of the study, the participants in the study are assessed using the Bradan scale and the entry score is recorded. Then, the training is carried out by the researcher based on the prevention of compression wound for 4 sessions of 1 hour in 4 days. And the rehabilitation team's actions are carried out according to the needs of the patient and the family in their presence and after three weeks the Risk rating of pressure ulcers is measured and the score is compared before and after the intervention.

Description
Patients who are at risk for pressure ulcers and other criteria (score less than 18 based on Brad's scale) are included in the study. At first, a meeting will be held with the presence of members of the rehab team to explain how the work process will work and how and to what extent, and the stages of the research. For 4 sessions of 1 hour, during 4 days, training was given to the patient and the family based on knowledge of compression, moisture and friction, changes in position and displacement, nutrition, and medication, and during the following days until the end of the study, the patient was given the training in terms of training. It is checked for the needs of the patient, and the rehabilitation team (consisting of the patient and family from the nurse, occupational therapist, physiotherapist, psychologist, psychiatrist, physician, social worker) is formed for the patient.
