The aim of this single group clinical trial is to investigate the effect of using normal saline in endotracheal suctioning at the ventilation status. A total of 40 patients, hospitalized in cardiac ICU after coronary artery bypass graft in Imam Ali hospital, are recruited. After transmission to ICU, firstly endotracheal suctioning without normal saline and two hours later endotracheal suctioning with instillation of 5cc normal saline in endotracheal tube is done. Before each suctiocing procedure, firstly arterial blood gases analysis, end tidal CO2 by capnograph and saturation O2 by palsoxymeter, peak inspiratory pressure, plateau pressure, and other parameters by monitoring ventilator are recorded. Then, dynamic compliance, static compliance and airway resistance are calculated and the patient is hyperoxygenized for 2 minutes and then the patient is disconnected from the ventilator and endotracheal suctioning is done by 12Fr catether with pressure of 120-150mmHg for 10-15 seconds. The patient is connected to the ventilator immediately after suctioning and arterial blood gases analysis and other desired respiratory parameters are measured again five and 15 minutes after connecting.

Inclusion criteria: elective coronary artery bypass graft, age less than 65, normal pre-operative pulmonary function tests, no chronic obstructive pulmonary disease, normal arterial blood gases in room air and before induction , Ejection fraction more than 40%, stable hemodynamic status after transmission to ICU, normal arterial blood gases after transmission to ICU, under ventilation by bennet 760
Exclusion criteria: abnormal arterial blood gases, hemodynamic disturbance, smoking
