evaluating the Clinical effects of mechanical debridement with adjunctive photodynamic therapy or chlorhexidine irrigation in peri implantitis

single blind,parallel, randomized clinical trial with 26 participants divided in control and treatment groups with sealed envelope randomization method.

patients will be selected from those referring to implant department, faculty of dentistry, islamic azad university of tehran
this clinical trial will be single blinded(patients)

Participants/Inclusion and exclusion criteria

inclusion criteria: 18-80 years of age, presence of at least one site with peri implantitis and screw type implants with rough surface which had been in function for at least 1 year
exclusion criteria: usage of antibiotics or NSAIDs in last 3 months, implant mobility, pregnancy or nursing, history of peri implantitis or periodontal treatment in last 6 months, uncontrolled diabetes, tobacco use and history of autoimmune diseases, malignancy, cardiovascular insufficiency or acute diseases of other organs

Intervention groups

Periodontal treatment comprising scaling and root planing (SRP) and cleaning of the implant surfaces with prophylaxis brush and paste will be
accomplished for each patient. 2 weeks after SRP, clinical indices will be recorded and patients will be randomly divided into two groups and treated with photodynamic therapy(810nm diode laser,0.3 W, 90s with indocyanine green) or chlorhexidine irrigation(control) with blunt needle. 2 weeks later these treatments will be repeated for control and treatment groups. chlorhexidine mouthwash(0.2%, 2 times per day and each time 10ml) will be prescribed for both groups for 4 weeks. clinical indices will be evaluated at 1,3 and 6 month after treatment.

Main outcome variables

probing pocket depth(PPD), clinical attachment loss(CAL), papillary bleeding index(PBI) and Plaque index (modified mombelli)
