Condition 5: Injury of blood vessels at forearm level. Condition 6: Injury of muscle and tendon at forearm level. Condition 7: Crushing injury of forearm. Condition 8: Other and unspecified injuries of forearm. Condition 9: Superficial injury of wrist and hand. Condition 10: Open wound of wrist and hand. Condition 11: Fracture at wrist and hand level. Condition 12: Dislocation, sprain and strain of joints and ligaments at wrist and hand level. Condition 13: Injury of blood vessels at wrist and hand level. Intervention 1: systemic analgesia: midazolam with a dose of 0.04mg/kg and phentanyl with a dose of 2 micrograms/kg. Intervention 2: Bier block: 30-50 of lidocaine solution with a dose of 0.5% is injected into local venous circulation during 7 minutes.
