The objective of this study is to assess of “ The effect of need based education on illness perception of cardiac patients”. Inclusion criteria is included patients with acute and chronic heart disease, stable condition, age between 18-60 years, the desire to participate in the research, ability to read and write, access to the phone after hospital discharge. Exclusion criteria is included patient's unwillingness to continue to cooperate, exacerbation and incidence of disabilities or life-threatening conditions. The study population included patients with acute and chronic heart disease that admitted in Shiraz University of Medical Sciences CCUs . The sample size is 100 people. The intervention consisted of face to face education and telephone follow up that each of them will be done in two sections and start after stability in the ward and finish one week after hospital discharge. According to the assessment of the patient’s illness perception, the primary result of the study is change in the patient’s illness perception.

Description
The intervention group: first needs assessment form is given to patients, after identifying the needs, face to face education for 30 to 45 minutes in two sessions will done. then follow up by telephone in two sessions, for 30 to 45 minutes will done. Educations is done with an emphasis on the educational needs of cardiac patients
