The aim of this study is evaluation of the effect of physician prescribed information intervention among 120 heart failure patients hospitalized in Shahid Madani heart hospital from December to February of 2013. Sampling will be done randomly.Study will be single blinded, data assessor will be blind.The subjects in control group will receive routine information given by doctor or nurse but the subjects in intervention group will receive physician prescribed health information in written form plus oral information given by librarian in addition to routine information . Written information will be prescribed by doctor and will be supplied by a librarian after necessary value adding services like translating and simplifying and appraising.Then it will be given to patients in intervention group following to the doctor approval.Outcomes of this study are as follows:
the effect of physician prescribed information intervention in readmission ; the effect of physician prescribed information intervention in reduction of treatment expenses; side effect of presentation physician prescribed information intervention(like:stress); the effect of physician prescribed information intervention in amount of drug and doses usage

Description
In this study intervention type is information prescribed by physician in the form of written information. Patients in the intervention group receive physician prescribed information in written format along with oral explanation about the same information presented by librarian.The content of information is about drugs, tests, exercise, symptoms , self care. The control group do not receive the written information and the librarian's oral explanation.
