Protocol summary
This study will be a randomized triple blind three-way cross-over trial which aims to investigate the effect of replacing the regular oil consumption of patients with type 2 diabetes with sesame-canola (a blend of sesame and canola oil) (SCO) in comparison with sesame oil (SO) and canola oil (CO) on blood pressure and cardiometabolic markers. Patients with type 2 diabetes will be selected to be entered into the study based on the inclusion criteria. All participants will receive a dietary prescription and recommendation to maintain a healthy diet during the study period. Then, they will be entered to a four week run-in period in which sunflower oil will be provided for regular oil consumption. After run-in period participants will be entered into the main phase of the study. Each intervention period will last for nine weeks, four weeks apart as wash-out period (in which sunflower oil will be provided). The participants will be randomized using statistical package for social sciences (SPSS) to receive SCO, SO or CO in each intervention period. SCO, SO, and CO will be provided for participants in the same packages labeled by a person who is not aware of the study objectives and protocol. Participants and personnel will be blinded to the intervention oils up to the end of the study period and analysis of the study results. Physical activity assessment (using three-day record), three-day weighted food record, anthropometric (including body weight, height, waist circumference and body fat percent) and blood pressure measurement will be conducted at the entrance, start, and the end of each intervention period. Blood samples (for assessment of lipid profile and glucose control markers) will also be taken from each participant at the start and the end of each intervention period. The change in lipid profile and blood glucose control markers will be compared between periods of SCO, SO and CO consumption, in crude and multivariable adjusted models.

مطالعه حاضر، یک کارازمایی بالینی تصادفی شده سه سو کور و متقاطع سه طرفه میباشد؛ که با هدف بررسی اثر جایگزینی روغن مصرفی معمول افراد دیابتی با روغن کنجد-کانولا( مخلوطی از روغن کنجد و کانولا) در مقایسه با روغن کنجد و روغن کانولا بر روی فشار خون و مارکرهای قلبی-عروقی، طراحی شده است. بیماران دیابتی نوع دو بر اساس معیارهای ورود، انتخاب و وارد مطالعه خواهند شد. تمام این بیماران توصیه های غذایی به منظور حفظ یک رژیم نرمال در طول مطالعه را دریافت خواهند کرد و سپس به مدت چهار هفته رابه عنوان دوره ورود به مطالعه با مصرف روغن آفتاب گردان خواهند گذراند و بعد از این مرحله وارد فاز اصلی مطالعه خواهند شد.
دوره های مداخله این مطالعه نه هفته ای خواهد بود که توسط چهار هفته شست و شو ( که روغن آفتاب گردان مصرف خواهد شد) از یکدیگر جدا میشوند. افراد با استفاده از نرم افزار SPSS بر حسب تصادف در هر دوره یکی از سه روغن کنجد، کانولا و کنجد-کانولا را دریافت خواهند کرد. در این مطالعه، هر سه نوع روغن در بسته بندی های یکسان و بدون برچسب و توسط فردی که نسبت به اهداف و شیوه اجرای مطالعه آگاهی ندارد، برای شرکت کنندگان تهیه و همچنین شرکت کنندگان و پرسنل مطالعه و نسبت به نوع روغن مصرفی و تا پایان مطالعه و تحلیل نتایج مطالعه کورسازی خواهند شد.
فعالیت بدنی (با استفاده از ثبت سه روزه)، ثبت سه روزه ی وزن غذا، اطلاعات تن سنجی (شامل وزن، قد، دور کمر و درصد چربی بدن) و فشارخون در ابتدا، انتها و میانه هر دوره مداخله ارزیابی و اندازه گیری خواهد شد و همچنین در آغاز و پایان هر دوره مداخله، نمونه خون (به منظور ارزیابی چربی خون و مارکرهای کنترل قند خون) از هر شرکت کننده گرفته می شود.
در نهایت تغییرات پروفایل لیپیدی و مارکرهای کنترل قند خون بیماران در بین دوره های مصرف سه نوع روغن کنجد، کانولا و کنجد-کانولا، به صورت خام و مدل تعدیل یافته چند متغیره مورد بررسی قرار خواهد گرفت.

Protocol summary
Study aim
To investigate the effect of replacing the regular oil consumption of participant (people with diabetes and their spouses) with sesame-canola (a blend of sesame and canola oil) in comparison with sesame oil and canola oil on blood pressure and cardiometabolic markers.
Design
Three-way randomized cross-over clinical trial
Settings and conduct
This study will be conducted at diabetes research center. All three types of oils will be provided for participants in the same packages labeled by a person who is not aware of the study objectives and protocol. Participants and personnel will be blinded to the intervention oil. Physical activity assessment, three-day weighted food record, anthropometric and blood pressure measurement will be conducted at the entrance, start, and the end of each intervention period and blood samples will also be taken from each participant (people with diabetes and their spouses) at the start and the end of each intervention period.
Participants/Inclusion and exclusion criteria
Patients with type 2 diabetes mellitus aged 18-60 years will be entered into the present study. each participant will enter the study with his/her spouse. Participants on insulin therapy or a history of cardiovascular, kidney and liver diseases, and cancer will not be entered to the study.
Intervention groups
Each intervention period will last for nine weeks, four weeks apart as wash-out period (in which sunflower oil will be provided). The participants will be randomized using a software to receive sesame, canola and sesame-canola oils in each intervention period.
Main outcome variables
Serum total cholesterol , triglyceride , high density lipoprotein cholesterol, low Density Lipoprotein cholesterol, and fasting blood sugar

Inclusion criteria:
Adults aged between 18-60 years old
With clinically diagnosed type 2 diabetes for at least 6-month and maximum of 10 years
With hemoglubin A1C lower than 8 percent
Only take oral anti-hyperglycemic agents for the treatment
have intention to enter the study and fill the informed consent
Exclusion criteria:
Being on insulin therapy
History of known Cardiovascular Diseases (Coronary Artery Disease, Stroke, Congestive heart disease)
Coronary artery bypass grafting (CABG)
History of kidney or liver disease [alanine aminotransferase (ALT) and aspartate aminotransferase (AST) three times more than normal values]
history of cancer
Changing the dose of lipid lowering medications for at least 3 months prior starting the study
being pregnant

As a three-way randomized controlled clinical trial the eligible participants will be randomly assigned to one of six following rolling methods using the statistical package for social sciences software (SPSS): 1) Sesame-canola, sesame, canola; 2) Sesame-canola, canola, sesame; 3)Canola, sesame-canola, sesame; 4)Canola, sesame, sesame-canola; 5)Sesame, sesame-canola, canola and 6)Sesame, canola, sesame-canola.

In this study, blinding will be done for treatment oils for both researchers and participants. The oils will be given in the same 2.5-liter bottles and will be coded for each oils (canola, sesame and sesame-canola) by someone who are not enroll in the study design .
