Comparing the effect of Phlebotomy and Cyproterone Compound on insulin resistance, anti-mullerian hormone, and Prostatic Specific Antigen (PSA) concentrations in patients with Polycystic Ovary Syndrome (PCOS), A cross over study.
After excluding cases with diabetes mellitus, Iron deficiency anemia, pregnancy, cardiovascular disease, hypertension, thyroid disorders and cancer, One hundred and twenty subjects with PCOS aged 18-45 years, referred to Reproductive Endocrinology Research Center are screened with CBC, Ferritin and 75 gram Oral Glucose Tolerance Test (OGTT), to be enrolled for the study and after comprehensive evaluation and completion of questionnaires, participants are randomized into two groups: phlebotomy and Cyproterone Compound.
In the phlebotomy group 450 ml of whole blood is taken and in the second group, Cyproterone compound are administrated for three cycles.
After three months, participants are re-evaluated and second blood sample for hormonal assays are taken, after six weeks of washout period, two groups are interchanged (i.e., Cyproterone Compound to phlebotomy and vice versa).
After three months, clinical and biochemical assessments are repeated and at the end of six months, any changes of clinical manifestations and or biochemical findings, will be compared between two groups.

ورود، وارد مطالعه می شوند. پس از تکمیل پرسشنامه به طور تصادفی در دو گروه قرص وفلبوتومی قرار می گیرند . گروه قرص برای سه دوره قرص سیپروترون کمپوند مصرف می کنند و گروه فلبوتومی برای فصد یک واحد خون کامل معادل 450 سی سی ارجاع داده می شوند .
بعد از سه ماه مجددا برای بیماران مراحل معاینات بالینی ، پرسشنامه ونمونه خون هورمونی مرحله دوم انجام می شود .و به مدت شش هفته زمان wash out در نظر گرفته شده وپس از شش هفته جای گروهها عوض می شود .یعنی گروه قرص برا ی فلبوتومی فرستاده میشود و برای گروه فلبوتومی
قرص سیپروترون کمپوند تجویز می شود. و بعد از سه ماه مجددا معاینه ،پرسشنامه و برسی بیو شیمیایی انجام می شود .
در خاتمه شش ماه تغییرات در علایم بالینی و معیار های آزمایشگاهی را در هر گروه قبل از شروع مداخله وبعد از هر دوره مداخله مقایسه خواهیم کرد.

مداخله ی اول فلبوتومی:گروهی که به طور تصادفی در گروه فلبوتومی قرار گرفته اند برای فصد خون به میزان 450 سی سی فرستا ده می شوند.و توصیه می شود که برای مدت سه ماه هیچ داروی هورمونی یا ترکیبات آهن استفاده نکنند
گروه مداخله (گروه فلبوتومی) : گروهی که به طور تصادفی در گروه فلبوتومی قرار گرفته اند برای فصد خون به میزان 450 سی سی فرستاده می شوند و توصیه میشود که برای مدت سه ماه هیچ داروی هورمونی یا ترکیبات آهن استفاده نکنند

Intervention 2- Cyproterone Compound:
For these patients, three cycles of pills are prescribed. Each cycle includes 21 days of use and 7 days without pills. They should not use any other medications (e.g. hormone(s) or Iron-containing drugs).
After three months, women in both study groups will return for completion of the questionnaire and blood sampling. After six weeks of washout period, two groups are interchanged (i.e., Cyproterone Compound to phlebotomy and vice versa). The hormonal profiles and clinical manifestations of participants in these two groups are compared.
Control group: patient who randomly allocated in control group, will be received the 1 tab of cyproteron compound for 21 days (with 7 days free of tab) for 3 cycles.
Intervention 2- Cyproterone CompoundControl group: For these patientspatient who randomly allocated in control group, three cycleswill be received the 1 tab of pills are prescribed. Each cycle includescyproteron compound for 21 days of use and(with 7 days without pills. They should not use any other medications (e.g. hormone(sfree of tab) or Iron-containing drugs). After three months, women in both study groups will return for completion of the questionnaire and blood sampling3 cycles. After six weeks of washout period, two groups are interchanged (i.e., Cyproterone Compound to phlebotomy and vice versa). The hormonal profiles and clinical manifestations of participants in these two groups are compared.
مداخله دوم :تجویز قرص سیپروترون کمپوند
گروهی که به طور تصادفی در این گروه قرار گرفته اند از روز اول پریود سه دوره 21 روز ه این قرص را که از طرف پژوهشکده در اختیار آنها گذاشته شده مصرف می کنند بعد از هر دوره 21 روزه به مدت یک هفته قرص قطع میشود . در این دوره به بیمار توصیه می شود هیچ گونه داروی هورمونی یاترکیبات آهن مصرف نکند. ودر پایان سه ماه هر دو گروه برای تکمیل پرسشنامه ونمونه . خون مرحله بعدی مراجعه کنند.پس از شش هفته جای دو گروه عوض می شود .یعنی گروه قرص فلبوتومی شده و برعکس.
گروه کنترل: افرادی که بطور تصادفی در گروه کنترل قرار میگیرند از روز اول سیکل قاعدگی روزانه یک عدد قرص سیپروترون کاماپاند را تا روز 21 سیکل قاعدگی دریافت میکنند و سپس بعد از هفت روز وقفه مجددا روزانه یک عدد قرص استفاده میکنند و تا سه سیکل ادامه میدهند.
مداخله دوم گروه کنترل:تجویز قرص سیپروترون کمپوند گروهی افرادی که به طوربطور تصادفی در این گروه کنترل قرار گرفته اندمیگیرند از روز اول پریود سه دورهسیکل قاعدگی روزانه یک عدد قرص سیپروترون کاماپاند را تا روز 21 روز ه این قرص را که از طرف پژوهشکده در اختیار آنها گذاشته شده مصرف می کنندسیکل قاعدگی دریافت میکنند و سپس بعد از هر دوره 21 روزه به مدتهفت روز وقفه مجددا روزانه یک هفتهعدد قرص قطع میشود . در این دوره به بیمار توصیه می شود هیچ گونه داروی هورمونی یاترکیبات آهن مصرف نکند. ودر پایاناستفاده میکنند و تا سه ماه هر دو گروه برای تکمیل پرسشنامه ونمونه سیکل ادامه میدهند. خون مرحله بعدی مراجعه کنند.پس از شش هفته جای دو گروه عوض می شود .یعنی گروه قرص فلبوتومی شده و برعکس.

Study aim
Comparing the effect of Phlebotomy and Cyproterone Compound on insulin resistance, anti-mullerian hormone, and Prostatic Specific Antigen (PSA) concentrations in patients with PCOS
Design
After obtaining informed consent, the 120 included patients were randomly allocated in a 1:1 ratio to either the iron-reduction group of phlebotomy and the cyproterone compound group , using a random number table. they didn't received any medication.
Settings and conduct
All refereed PCOS patients to Endocrine clinic, Research Institute for Endocrine Sciences, Shahid Beheshti University of Medical Sciences were randomly allocated to phlebotomy and CC groups. Phlebotomy Patients undergo venesection of 450 ml of whole blood and are advised not to use any hormonal or iron- containing medications. Cyproterone Compound group were advised three cycles of pills are prescribed. Each cycle includes 21 days of use and 7 days without pills. They should not use any other medications (e.g. hormone(s) or Iron-containing drugs).
Participants/Inclusion and exclusion criteria
Inclusion criteria: PCOS pateints with: 18-45 years old; Hgb more than 12; ferritine more than 20; no diabetes melitus ; no heart problem; no hypertention or under treatment; not pregnant;no thyroid problem; no cancer. Exclusion criteria: pregnancy; heart or thyroid disease; hypertntion; cancer; diabetes melitus.
Intervention groups
RCT will be performed in two groups: The study group was undergone venesection of 450 mL of whole blood. The cyproteron compound group received Cyproterone Compound pill for 3 months from day 1 of spontaneous or progesterone-induced menstrual cycle onwards for 3 weeks and Followed by a pill-free interval of 7 days
