Protocol summary
Our aim is the comparison of anesthetic efficacy of Articaine versus Lidocaine in Inferior Alveolar Nerve Block injection plus Intraligamentary injection in mandibular molars with irreversible pulpitis. 60 patients will be participated in this study in Shahid Beheshti University of Medical Sciences. All the patients should have first or second vital mandibular molar without spontanous pain and positive response to Electrical Pulp Tester and prolonged response to the cold test. Before the anesthetic injections the patients will be asked to indicate their pain, in each stage, before and after the Inferior Alveolar Nerve Block and Intraligamentary Injections and while preparing access cavity and initial filing on Heft Parker Visual Analogue Scale. The patients will be divided randomly into 2 subgroups and will be received Lidocaine or Articaine. The pulse rate will be measured in each stage by pulse-oximeter. All the patients will be received 1.8 ml Inferior Alveolar Nerve Block injection of each anesthetic solution; 2% Lidocaine with epinephrine 1/80000 or 4% Articaine with epinephrine 1/100000. If the Electrical Pulp Tester number will be under 80; we will perform supplemental injection by the same solution. The access cavity will be prepared and initial filling will be done.The patients will indicate their pain in these two stages on Heft Parker Visual Analogue Scale. The success of 2 injections is the ability for preparing the access cavity and initial filling with no or mild pain.

هدف ما مقایسه اثربخشی بی حسی لیدوکایین در مقابل آرتیکایین در تزریق بلاک عصب آلوئولار تحتانی همراه با تزریق داخل لیگامانی در دندان های مولر ماندیبول با پالپیت برگشت ناپذیر است. 60 بیمار در دانشگاه علوم پزشکی شهید بهشتی در این مطالعه شرکت داده خواهند شد. تمامی بیماران باید دارای مولر اول یا دوم وایتال ماندیبول بدون درد خود به خودی و پاسخ مثبت به الکتریکال پالپ تستر و پاسخ مدت دار به تست سرما باشند. قبل از انجام تزریقات بی حسی از بیماران درخواست می شود تا میزان درد خود را در هر مرحله یعنی قبل و بعد از تزریق بلاک عصب آلوئولار تحتانی و تزریق داخل لیگامانی و ضمن تهیه حفره دسترسی و قرار دادن فایل اولیه بر روی Heft Parker Visual Analogue Scale مشخص نمایند. بیماران به دو گروه به صورت تصادفی تقسیم می شوند و لیدوکایین و یا آرتیکایین دریافت خواهند کرد. نبض بیماران در هر مرحله با پالس اکسی متر اندازه گیری می شود. همه بیماران 8/1 میلی لیتر از هر ماده بی حسی با لیدوکایین 2% با اپی نفرین یک هشتاد هزارم و یا آرتیکایین 4% با اپی نفرین یک صد هزارم برای تزریق بلاک عصب آلوئولار تحتانی دریافت خواهند کرد. اگر عدد الکتریکال پالپ تستر زیر 80 باشد؛ تزریق مکمل را با همان ماده انجام می دهیم. حفره دسترسی تهیه و فایلینگ اولیه انجام داده خواهد شد. موفقيت تركيب دو تزريق به معنای توانايی برای تهیه كردن حفره دسترسی و فایلینگ اولیه بدون درد و يا درد كم خواهد بود.

Inclusion criteria include:patients who a) have no systemic diseases, b) are 18 years old or older, c) have a first or second vital mandibular molar with no sign of periapical pathosis in radiographs, and d) do not take any pain killers 12 h prior to the treatment.Exclusion criteria comprise a) teeth with more than 0.5 mm mobility and more than 3 mm probing depth in their mesial or distal surfaces, b) teeth with no vital coronal pulp tissue on access, and c) patients with hypersensitivity to lidocaine or articaine, pregnancy or lactation.

All patients are divided into experimental groups using stratified permutations blocks randomization. Patients are stratified to 4 groups according to the gender and the type of the tooth (first or second mandibular molar). Then cases in every subgroup are randomly assigned into 2 groups of lidocaine and articaine with random block size of 4.

تمام بیماران با روش تصادفی سازی طبقه بندی شده بلوکی به دو گروه آزمایشی تقسیم میشوند. بیماران بر اساس جنسیت و نوع دندان( مولر اول یا دوم فک پایین) به چهار زیر گروه طبقه بندی میشوند.سپس بیماران در هر زیر گروه بصورت تصادفی با استفاده از بلوکهای چهارتایی به دو گروه لیدوکایین و آرتی کایین تخصیص داده میشوند.

Equal numbers of lidocaine and articaine cartridges, which are covered and given a code, are provided. To obtain allocation concealment, an assigned nurse is aware of the codes and, according to the groups (lidocaine or articaine), gives out the cartridges randomly. There is 1 code for each of the 2 cartridges packed together. One clinician completes the designated questionnaire and another one administers the injections. All the injections are given by the same clinician. Block and supplemental injections are done with the same anesthetic. Protocol summary
Study aim
The objective of this study was to compare the anesthetic efficacy of supplemental intraligamentary (PDL) injections of 4% articaine with 2% lidocaine in the first and second mandibular molars _diagnosed with irreversible pulpitis_ after an ineffective inferior alveolar nerve block (IANB), using the same anesthetic.
Design
Triple blind, consealed randomised, superiority, parallel group trial .A third phase study.
Settings and conduct
Endodontic Department, School of Dentistry,Shahid Beheshti University of Medical Sciences
Participants/Inclusion and exclusion criteria
Inclusion criteria include:patients who a) have no systemic diseases, b) are 18 years old or older, c) have a first or second vital mandibular molar with no sign of periapical pathosis in radiographs, and d) do not take any pain killers 12 h prior to the treatment.Exclusion criteria comprise a) teeth with more than 0.5 mm mobility and more than 3 mm probing depth in their mesial or distal surfaces, b) teeth with no vital coronal pulp tissue on access, and c) patients with hypersensitivity to lidocaine or articaine, pregnancy or lactation.
Intervention groups
1- Men and woman with a first or second mandibular molar who anesthetize with lidocaine
2-Men and women with a first or second mandibular molar who anesthetize with articaine
Main outcome variables
anesthetic efficacy of supplemental intraligamentary (PDL) injections of 4% articaine with 2% lidocaine in the first and second mandibular molars _diagnosed with irreversible pulpitis_ after an ineffective inferior alveolar nerve block (IANB), using the same anesthetic.

Randomization description
All patients are divided into experimental groups using stratified permutations blocks randomization. Patients are stratified to 4 groups according to the gender and the type of the tooth (first or second mandibular molar). Then cases in every subgroup are randomly assigned into 2 groups of lidocaine and articaine with random block size of 4.

Blinding description
Equal numbers of lidocaine and articaine cartridges, which are covered and given a code, are provided. To obtain allocation concealment, an assigned nurse is aware of the codes and, according to the groups (lidocaine or articaine), gives out the cartridges randomly. There is 1 code for each of the 2 cartridges packed together. One clinician completes the designated questionnaire and another one administers the injections. All the injections are given by the same clinician. Block and supplemental injections are done with the same anesthetic. This is a triple blind study; which means that the clinician who gives the injection, the patients, outcome assessor and the statistician are not aware of the anesthetic.
