Objective: The effect of dry needling of tenar muscles on the management of patients with carpal tunnel syndrome.
Inclusion criteria: patients who Phalen and Tinel tests are positive; patients electromyography (EMG) has a median reduction in NCV.
Exclusion criteria: presence of patients with symptoms of CTS; drugs that cause neuropathy; the pregnancy and breastfeeding, history of previous surgeries or injections Astrvyydal to the wrist, elbow and wrist deformity tumor.
The study population and sample size: sample size of the study population to patients referred to physiotherapy centers in Semnan University of Medical Sciences, in March 2016 to March 2017 who were referred by physicians specializing in the diagnosis of carpal tunnel syndrome, non-random sampling method was simple up to average about 100 patients.
Interventions of the study and its time and primary outcomes: The electrodiagnostic study includes a review of median nerve NCV (nerve conduction velocity) and distal latency time (distal latency) performed on patients. The Boston Questionnaire (BQ) to be filled by volunteers. As well as the functional operations of the maximum power of pinching and griping assessment and thenar of hypertrophy and Myvfshyal Myofascial trigger point pain (myofascial trigger point pain) are examined. Then the volunteers were randomized to receive treatment groups. In the intervention group DN, DN two sessions with an interval of two days of one another, occurs. To assess the reliability of the effect of treatment, all evaluations are done after two weeks.

معیار ورود: بیمارانی که تستهای Phalen و Tinelشان مثبت است؛ بیمارانی الکترومیوگرافی (EMG ) دارای کاهش سرعت هدایت عصب مدین.
معیار خروج : وجود بیماری با علائم CTS ؛ مصرف داروهایی که باعث نوروپاتی می شود؛ بارداری و شیردهی؛ سابقه جراحی های قبلی و یا تزریق استروئیدال به مچ دست؛ تومور یا دفورمیتی ناحیه آرنج و مچ دست.
جمعيت مورد مطالعه و حجم نمونه: جمعيت مورد مطالعه و حجم نمونه را بیماران مراجعه کننده به مراکز فیزیوتراپی دانشگاه علوم پزشکی سمنان، از اسفند 1394 لغایت 1395 که توسط پزشک متخصص با تشخیص سندرم تونل کارپ ارجاع شده اند، با روش نمونه گیری ساده غیرتصادفی بوده به طور میانگین حدود 100 بیمار تشکیل می دهند.
مداخله مورد مطالعه وزمان آن و پيامد هاي اوليه: ابتدا مطالعه ی الکترودیاگنوستیک عصب مدین شامل بررسی NCV(nerve conduction velocity) و زمان تاخیر انتهایی (distal latency) بر روی بیماران صورت می گیرد. سپس پرسشنامه بوستون(BQ) توسط داوطلبان پر می شود. همچنین عملکردهای فانکشنال دست شامل حداکثر قدرت pinching و griping ارزیابی می شود و عضلات تنار از لحاظ هایپرتروفی و وجود درد میوفشیال تریگرپوینت(myofascial trigger point pain) بررسی می شوند. پس از آن داوطلبان بصورت تصادفی در دو گروه درمانی قرار می گیرند. در گروه تحت مداخله DN ، دو جلسه درمان با DN به فاصله ی دو روز از هم، صورت می گیرد. جهت بررسي پايايي اثر درمان، تمامي ارزيابي ها دو هفته بعد هم انجام مي شود.

Protocol summary
Study aim
The effect of dry needling of tenar muscles on the management of patients with carpal tunnel syndrome.
Design
The clinical trial with a control group, with parallel groups, randomized, single-center. The sample size is 12 patients (consist of 2 groups of 50 control and intervention).
Settings and conduct
Physiotherapy clinics, Semnan University of Medical Sciences
Participants/Inclusion and exclusion criteria
Inclusion criteria: patients who Phalen and Tinel tests are positive; patients electromyography (EMG) has a median reduction in NCV. Exclusion criteria: presence of patients with symptoms of CTS; drugs that cause neuropathy; the pregnancy and breastfeeding, history of previous surgeries or injections Astrvyydal to the wrist, elbow and wrist deformity tumor.
Intervention groups
]ntervention group: In the intervention group DN, DN two sessions with an interval of two days of one another, occurs. In both groups immediately after the second meeting with the re-evaluation electrodiagnostic tests of functional performance and completion of a questionnaire by volunteers Boston is repeated. To assess the reliability of the effect of treatment, all evaluations are done after two weeks.
Control group: There is no intervention in the control group. In both groups immediately after the second meeting with the re-evaluation electrodiagnostic tests of functional performance and completion of a questionnaire by volunteers Boston is repeated.

Patients who test positive Phalen and Tinel them
patients in their study, electromyography (EMG) decreased median nerve conduction velocity has been confirmed
Patients who evaluate trigger Myofascial trigger points in the muscles Tnarshan according to diagnostic criteria simons been confirmed
Exclusion criteria:
The existence of a disease that mimics the symptoms of CTS such as polyneuropathy, radiculopathy, cervical, thoracic outlet syndrome and double-crush syndrome
There are conditions such as hyperthyroidism, gout, systemic lupus erythematosus, rheumatoid arthritis, diabetes, chronic kidney failure and acromegaly
