  • Protocol summary

    The present study aims to assess the therapeutic effect of the Bilinaster drop on jaundice in infants undergoing photo therapy. The study population consists of all infants with jaundice. The present randomized double-blind clinical trial is conducted on two groups of infants; Group 1: The infant with jaundice receive placebo. Group 2: The infant with jaundice receive Bilinaster drop. The sample size is determined using mean difference formula as 100 infants in each group, making a total sample size of 200 infants. Conduct and randomization: Bilinaster and placebo drops (medication and placebo) are produced by Sobhan Pharmaceutical Company in similar bottles and packing. placebo contains the same product like bilineaster but without manna. A computer software program is used to produce random numbers to allocate the medication and placebo to two groups. Adequate medication and placebo for each group will be placed in a special envelope marked with patient number and medication code. Thus, patients, doctor and assessor will have no knowledge of the nature of medication or group allocation. Infants will enter the present study according to inclusion criteria and receive their particular medications. The study setting will be in Bu Alisina Hospital in Sari. This study is done after parents consent. Inclusion criteria include all infants present with jaundice. Exclusion criteria include being having incompatible blood group (RH), suffering from fauvism, being preterm, needing blood transfusion, suffering from infection or being the case of hemolysis.

    هدف از انجام این مطالعه بررسی اثر درمانی مصرف قطره بیلی ناستر ساخته شده توسط شرکت سبحان دارو در زردی نوزادان تحت درمان با فتوتراپی است. جمعیت مورد مطالعه شامل کلیه نوزادان دچار زردی هستند. این مطالعه يك كار آزمايي باليني تصادفی دو سو کوراست که پس از گرفتن رشایت نامه از والدین دردو گروه قرار مي گيرند: در گروه اول نوزاد دچار زردی قطره بیلی ناستر به همراه فتوتراپی دریافت می کند. در گروه دوم نوزاد دچار زردی تحت درمان با فتو تراپی، پلاسبو نیز دریافت می کنند. حجم نمونه با استفاده از فرمول حجم نمونه تفاضل میانگین درهر گروه به تعداد 50 نوزاد و جمعاً 100 نوزاد محاسبه شد. کور سازی و تصادفی سازی: در این مطالعه، قطره بیلی ناسترو قطره پلاسبو (دارو و دارو نما) در بسته بندی و شیشه داروی کاملا مشابه توسط شرکت سبحان دارو تولید شده است. محتوئ پلاسبو مانند بیلی ناستر است و فقط شیر خشت ندارد.به منظور مراعات تخصیص تصادفی با استفاده از برنامه کامپیوتری اعداد تصادفی استخراج و از آنها کدهایی براساس بلوک بندی نوزادان به2گروه تهیه شده و بر روی دارو و دارونما نصب میگردد. سپس برای هر گروه دارو و دارو نما داخل پاکت مخصوص گذاشته شده و روی پاکت شماره بیمار و کد مربوط به دارو درج می گردد. لذا بیمار و پزشک و ارزیابی کننده از ماهیت دارو و نوع گروه بدون اطلاع خواهند بود. در نهایت نوزادان بر اساس معیارهای ورود وارد مطالعه می شوند و پاکت داروی مخصوص خود را دریافت می کنند پرسشنامه مربوط به مادر و نوزاد پس از تکمیل توسط مسئول مربوطه، جهت انالیز وارد کامپیوتر می گردد. مرکز مورد مطالعه بیمارستان بوعلی ساری می باشد. شرایط ورود عبارت است از کلیه نوزادانی که با زردی مراجعه می کنند شرایط خروج عبارت است از نوزادان با ناسازگاری گروه های خونی یا ارهاش، فاویسم، نارسی، نیاز به تعویض خون، ابتلا به هولیز یا عفونت

    Control group: The neonate with jaundice will receive placebo. The number of placebo drops per day is similar to the number of bilinaster drops. Intervention Group: The neonate with jaundice will receive Bilineaster drop which is manufactured by Sobhan Daru Company and is included manna of cotoneaster ( Shire-khesht). Bilineaster drop is prescribed about 1 g manna per day (15 drop) that is divided into 3 doses and at most until 5 days .

    For blinding in this trial, similarity of interventions in the two treatment groups under study was anticipated. The placebo was prepared in similar packages, with all the same physical and similar properties (in terms of color, odor, consistency, etc.). The amount of ingestion and the number of times a day was the same in both products. The only difference in these two products was the code inserted (numerical code) on the package and the corresponding box. As the mothers and prescriber (nurse) and laboratory technician were not aware of the nature of these two products and the codes entered.

    برای اعمال کورسازی در این کارآزمایی، مشابهت مداخلات در دو گروه درمانی تحت مطالعه، پیش بینی شده بود. دارونما در بسته‏ های مشابه، با کلیه خواص فیزیکی و ظاهری مشابه (از نظر رنگ، بو، قوام ظاهری و ...) تهیه شد. مقدار مصرف و تعداد بار مصرف در روز نیز در هر دو فراورده، یکسان بود. تنها تفاوت در این دو فراورده، کد درج شده (کد عددی) بر روی بسته و جعبه مربوطه بود. بطوریکه مادران و تجویز کننده دارو (پرستار) و تکنسین آزمایشگاه از ماهیت این 2 فرآورده و کدهای درج شده مطلع نبودند.

    Control group: The infant with jaundice will receive placebo. The infants will receive 5 drops of placebo per kg, every 8 hours for 5 days.
    Control group: The neonate with jaundice will receive placebo. The number of placebo drops per day is similar to the number of bilinaster drops.

Study aim

Inclusion criteria: All term neonate with jaundice weighted between 2500 -4000 gr and fed with breast milk. Exclusion criteria include babies with the incompatibility of blood groups or RH, suffering from fauvism, being preterm, needing blood transfusion, hemolysis or infection
Intervention groups
Control group: The neonate with jaundice will receive placebo. The number of placebo drops per day is similar to the number of bilinaster drops. Intervention Group: The neonate with jaundice will receive Bilineaster drop which is manufactured by Sobhan Daru Company and is included manna of cotoneaster ( Shire-khesht). Bilineaster drop is prescribed about 1 g manna per day (15 drop) that is divided into 3 doses and at most until 5 days .

Randomization description
A computer software program is used to produce random numbers to allocate the medication and placebo to two groups. Adequate medication and placebo for each group will be placed in a special envelope marked with patient number and medication code. Thus, patients, doctor and assessor will have no knowledge of the nature of medication or group allocation

Blinding description
For blinding in this trial, similarity of interventions in the two treatment groups under study was anticipated. The placebo was prepared in similar packages, with all the same physical and similar properties (in terms of color, odor, consistency, etc.). The amount of ingestion and the number of times a day was the same in both products. The only difference in these two products was the code inserted (numerical code) on the package and the corresponding box. As the mothers and prescriber (nurse) and laboratory technician were not aware of the nature of these two products and the codes entered.
