Protocol summary
Objectives: In this study we will evaluate the effect of cognitive behavioral therapy on the serum TNF-α and IL-8 level of fibromyalgia patients to find out if this therapy could decrease the pain mediators in this patients or not.
Design: Randomized controlled trial.
Setting and conduct: The serum level of TNF-α and IL-8 will be assessed before and after the performance of cognitive behavioral therapy and the changes will be evaluated.
Inclusion criteria: The diagnosis of fibromyalgia according to the criteria of American college of Rheumatology (2010)
Exclusion criteria: Vitamin D deficiency; Metabolic disease; Diabetes Mellitus; mental illnesses; Other rheumatologic disorders; hepatic diseases; chronic renal disorders; heart diseases; Malnutrition; and cancer.
Intervention: After identification of fibromyalgia patients interested in non-pharmacological treatment of the disease, cognitive behavioral therapy will be performed in 10 sessions (once a week) for intervention group. The maximum number of patients in each group will be five patients. The control group (patients in the waiting list for cognitive behavioral therapy) will receive no therapy.
Main outcome measures: TNF-α and IL-8.

مداخلات:پس از شناسایی بیماران علاقه مند به درمان غیردارویی، درمان شناختی-رفتاری در 10 جلسه (هفته ای یک جلسه) برای بیماران به صورت گروهی انجام می شود. در هر گروه حداکثر 5 بیمار وارد می شوند. ّبیماران گروه کنترل ( بیمارانی که در لیست انتظار درمان شناختی-رفتاری قرار دارند) هیچ درمانی نخواهند گرفت.

The fibromyalgia patients will be divided into the 3 study groups. For the first group, cognitive behavioral therapy will be performed in 20 sessions (once a week). Protocol summary
Study aim
In this study the effect of cognitive behavioral therapy on the serum TNF-α, IL-8, and IL-6 level of fibromyalgia patients will be evaluated and compared with the effect of medication (duloxetine) and no treatment.
Design
Clinical trial with control group, parallel groups, single blinded, randomized.
Settings and conduct
In this study which will be performed in Shafa Orthopedic Hospital, the serum level of TNF-α, IL-8, and IL-6 will be assessed before and after the intervention in three groups and the changes will be evaluated. The intervention type will be concealed form the main investigator who will do the tests and analyse the data.
Participants/Inclusion and exclusion criteria
Inclusion criteria: The diagnosis of fibromyalgia according to the criteria of American college of Rheumatology (2010)
Exclusion criteria: Vitamin D deficiency; No previous history of fibromyalgia treatment; Metabolic disease; Diabetes Mellitus; mental illnesses; Other rheumatologic disorders; hepatic diseases; chronic renal disorders; heart diseases; Malnutrition; and cancer.
Intervention groups
The fibromyalgia patients will be divided into the 3 study groups. For the first group, cognitive behavioral therapy will be performed in 20 sessions (once a week). The maximum number of patients in each group will be five patients. For the second group, deloxetine (60 mg/day) will be administered for 5 months. The control group will receive no therapy.

Serum level of TNF-α, IL-8, and IL-6.
