    Effect of platelet lysate (PL) and platelet-rich plasma (PRP) on chronic knee osteoarthritis (OA)
    بررسی اثر بخشی لیزات پلاکتی بر استئوآرتریت مزمن زانو
    بررسی اثر بخشی لیزات پلاکتی و پلاسما غنی از پلاکت بر استئوآرتریت مزمن زانو
    40 patients were selected based on radiologic findings with a period of 3 months symptoms of knee osteoarthritis and classified into two groups of PL and control group without any treatment. For the preparation of PL, 20 ml of blood from the upper cubital vein will be taken and, after production of PRP and, in order to produce PL, it is frozen at -80 ° C for 60 minutes, and then at 37°C Melt for 15 minutes. Then, centrifuge the platelets and WBC to remove the membranes and filter with 0.22 micrometers.

    تعداد 40 بیمار در بر اساس یافته¬های رادیولوژی با دوره علائم سه ماهه آرتروز زانو انتخاب شده و در 2 گروه درمان با PL و گروه کنترل بدون هیچ گونه درمانی طبقه بندی می شوند. برای تهیه PL از به میزان 20ml خون از ورید کوبیتال اندام فوقانی گرفته خواهد شد و بعد از سانتریفوژ PRP جدا شده و به منظور تولید PL آن را در دمای -80 درجه سانتی گراد به مدت 60 دقیقه منجمد کرده و سپس در دمای 37 درجه سانتی گراد به مدت 15 دقیقه ذوب میکنیم. پس از آن به منظور حذف غشا های پلاکتی و گلبول های سفید خون آن را سانتریفیوژ کرده و با فیلتر 0.22 میکرومتر فیلتر میکنیم.
    تعداد 25 بیمار در بر اساس یافته های رادیولوژی با دوره علائم سه ماهه آرتروز زانو انتخاب شده و در زانو چپ از لیزات پلاکتی و در زانو راست از پلاسمای غنی از پلاکت استفاده شد. برای تهیه لیزات پلاکتی از به میزان 20ml خون از ورید کوبیتال اندام فوقانی گرفته خواهد شد و بعد از سانتریفوژ پلاسمای غنی از پلاکت جدا شده و به منظور تولید لیزات پلاکتی آن را در دمای -80 درجه سانتی گراد به مدت 60 دقیقه منجمد کرده و سپس در دمای 37 درجه سانتی گراد به مدت 15 دقیقه ذوب میکنیم. پس از آن به منظور حذف غشا های پلاکتی و گلبول های سفید خون آن را سانتریفیوژ کرده و با فیلتر 0.22 میکرومتر فیلتر میکنیم.
    The following items for the patients who are most relevant, recorded: 1) Pain by visual analog scale (VAS) 2) Knee function by the WOMAC 3) Knee function by KOOS 4)The thickness of the knee cartilage using MRI After PL and quality control, patients will be referred to the Rehabilitation and Physical Medicine Department of the Shahid Madani Hospital for the injection of PL in the knee. An injection of PL into patients will be done by supralateral method, in which the patient will rest on the back and the knee will almost completely open. A second injection is repeated at intervals of 21 days. Then, in order to evaluate the effect of PL on patients in the intervals of 1, 3, 6 and 12 months after the first injection, the cases mentioned above will be recorded.

    در ابتدا موارد زیر برای این بیمارانی که دارای معیارهای ورود ذکر شده هستند، ثبت میشود: 1) درد به وسیله مقیاس آنالوگ نظری(VAS) 2) عملکرد زانو به وسیله ایندکس آرتریت دانشگاه McMastero و Western Ontorio (WOMAC) 3) عملکرد زانو به وسیله مقیاس امتیاز بندی نتایج استئوارتریت و آسیب زانو (KOOS) 4) میزان ضخامت غضروف زانو با استفاده از MRI پس از تهیه PL و کنترل کیفی آن، بیماران به بخش توانبخشی و طب فیزیکی بیمارستان شهید مدنی جهت تزریق PL در زانو ارجاع داده خواهند شد. تزریق PL به بیماران به روش supralateral انجام خواهد شد که در آن بیمار به پشت خوابیده و زانو تقریبا به طور کامل باز می¬شود. تزریق دوبار دیگر با فاصله 21 روز در میان تکرار میشود. سپس برای بررسی میزان تاثیر PL بر بیماران در بازه های زمانی 1 و3 و6 و 12 ماه پس از اولین تزریق مواردی که قبلا ذکر شد ثبت خواهد شد.
    در ابتدا موارد زیر برای این بیمارانی که دارای معیارهای ورود ذکر شده هستند، ثبت میشود: 1)درد به وسیله مقیاس آنالوگ بصری 2)عملکرد زانو به وسیله ایندکس آرتریت دانشگاه (WOMAC) 3) میزان ضخامت غضروف زانو با استفاده از ام آر آی پس از تهیه لیزات پلاکتی و پلاسمای غنی از پلاکت و کنترل کیفی آنها، بیماران به بخش توانبخشی و طب فیزیکی بیمارستان شهید مدنی جهت تزریق در زانو ارجاع داده خواهند شد. تزریق لیزات پلاکتی و پلاسمای غنی از پلاکت به بیماران به روش فوق جانبی انجام خواهد شد که در آن بیمار به پشت خوابیده و زانو تقریبا به طور کامل باز می شود. تزریق دوبار دیگر با فاصله 21 روز در میان تکرار میشود. سپس برای بررسی میزان تاثیر لیزات پلاکتی و پلاسمای غنی از پلاکت بر بیماران در بازه های زمانی 1 و6 ماه پس از اولین تزریق مواردی که قبلا ذکر شد ثبت خواهد شد.

    زنان باردار یا زنانی که در دوران شیردهی
    افراد دارای بدخیمی،فراد با بیماری های قلبی شدید، دیابت شیرین کنترل نشده، آرتریت روماتوئید، بیماری های خونریزی دهنده، سابقه کم خونی، نقرص، فیبرومیالژی و سندرم خستگی مزمن
    افراد وابسته به استامینوفن یا ویکودین یا سابقه استفاده نادرست از دارو
    سابقه تزریق کورتیزون در 6 هفته گذشته
    استفاده از داروهای ضد التهابی غیر استروییدی از 1هفته قبل
    داشتن هموگلوبین کمتر از 11 g/dlو پلاکت کمتر از 150000/mµ
    داشتن و استفاده از دارو های ممانعت کننده از تجمع پلاکتی و یاضد انعقادی مانند هپارین
    سابقه جراحی زانو در 3ماه گذشته
    دفورماسیون خارج مفصلی شدید (varus >5ᵒ, valgus >5ᵒ)
    افراد زیر 18سال و بالای 75 سال
    زنان باردار یا زنانی که در دوران شیردهی
    افراد دارای بدخیمی،فراد با بیماری های قلبی شدید، دیابت شیرین کنترل نشده، آرتریت روماتوئید، بیماری های خونریزی دهنده، سابقه کم خونی، نقرص، فیبرومیالژی و سندرم خستگی مزمن
    افراد وابسته به استامینوفن یا ویکودین یا سابقه استفاده نادرست از دارو
    سابقه تزریق کورتیزون در 6 هفته گذشته
    استفاده از داروهای ضد التهابی غیر استروییدی از 1هفته قبل

The following items for the patients who are most relevant, recorded: 1) Pain by visual analog scale (VAS) 2) Knee function by the WOMAC 3)The thickness of the knee cartilage using MRI After PL and PRP quality control, patients will be referred to the Rehabilitation and Physical Medicine Department of the Shahid Madani Hospital for the injection of PL and PRP in the knee. An injection of PL and PRP into patients will be done by supralateral method, in which the patient will rest on the back and the knee will almost completely open. A second injection is repeated at intervals of 21 days. Then, in order to evaluate the effect of PL and PRP on patients in the intervals of 1and 6 months after the first injection, the cases mentioned above will be recorded.
Participants/Inclusion and exclusion criteria
A total of 25 patients were selected based on radiologic findings with a period of three-month symptoms of knee OA.
Intervention groups
Treatment of left knee with PL Treatment of right knee with PRP
Main outcome variables
1- Determining knee pain by VAS in all phases of clinical trial 2-Determination of knee function by WOMAC in all phases of clinical trial 3-Determine the range of knee motion by manual joiniometry in all phases of the trial 4-Measuring Growth Factors in PL and PRP

Inclusion criteria:
Men and women between the ages of 18 and 75 with OA diagnosis based on the American College of Rheumatology Analog scale of knee pain (VAS) equal to or greater than 2.5 People with at least 4 months of history of pain or swelling in one or both knees Radiological classification scale Kellgren-Lawrence 1 or 2 The availability of individual during the study period BMI Between 20 and 35
Exclusion criteria:
People under the age of 18 and over 75 years Pregnant women or women who are breastfeeding People with malignancy,People with severe heart disease, uncontrolled diabetes mellitus, rheumatoid arthritis, hemorrhagic diseases, history of anemia, arthritis, fibromyalgia and chronic fatigue syndrome Those linked to acetaminophen or Vicodin or a history of drug misuse History of cortisone injections in the last 6 weeks The use of non-steroidal anti-inflammatory drugs 1 week ago Having hemoglobin less than 11 g / dl and platelet count less than 150000 / mμ The use of inhibitors of platelet aggregation and anti-coagulation such as heparin History of knee surgery in the last 3 months Extraordinary deformation (varus >5ᵒ, valgus >5ᵒ)
