Erectile dysfunction has a high prevalence and many harmful physical psychosomatic and psychosocial effects, and has a devastating impact on the quality of life of the patient and his sexual partner. Common therapies, in spite of the relative effect, are often sectional and therefore have no long-term and certain effects. Side effects and invasiveness of some methods are other disadvantages of common treatments. In this clinical trial, the impact of a new and less invasive treatment, called low-intensity shock waves (LI-ESWT), on 128 men over 40 years of age with erectile dysfunction with no response to phosphodiesterase type 5 inhibitors will be investigated.
Patients with a history of psychological disorders based on Beck's standard questionnaire, history of neurological diseases, history of previous radical prostatectomy and pelvic radiotherapy will be excluded from the study. At first, the standard Beck Questionnaire will be completed for all patients to exclude patients with psychological disorders. Then, the severity of erectile dysfunction in participants will be determined by the standard IIEF questionnaire. Afterwards, patients will be subjected to LI-ESWT once a week for six consecutive weeks by a single operator using the Richard wolf GmbH-Germany(Piezowave2) device. The energy density used is 0.160 mJ/mm2 and its frequency is 6HZ-8HZ and the total number is 6000 shock. Each time, 2000 waves of shock are applied to the corpus cavernosome of each side and 1000 waves to the curves of the two sides. After completion of treatment, patients will be asked by face-to-face interview or telephone for possible side effects. Again, the IIFF score will be calculated after the completion of the course, three and six months after treatment. The treatment success threshold will be considered to be at least 5 points increase in the IIEF questionnaire.

اختلال عملکرد نعوظی شیوع زیاد و آثار زیانبار جسمی و روانی متعددی دارد و نتایج مخربی درکیفیت زندگی بیمار و شریک جنسی اش میگذارد. درمان های رایج آن علیرغم تاثیر نسبی اغلب به صورت مقطعی بوده و بنابراین تاثیر بلندمدت و قطعی ندارند. عوارض جانبی و تهاجمی بودن برخی از روش ها از دیگر معایب درمان های رایج می باشند.
در این کار آزمایی بالینی تاثیر روش درمانی نوین و کم تهاجمی با عنوان امواج شوکی برون اندامی با شدت اندک (LI-ESWT) بر روی 128 مرد بالای 40 سال و مبتلا به اختلال عملکرد نعوظی با عدم پاسخ درمانی به مهارکنندگان فسفودی استراز نوع 5 بررسی می گردد. بیماران با سابقه اختلالات سایکولوژیک بر اساس پرسشنامه استاندارد Beck؛ سابقه بیماری های نورولوژیک؛ سابقه رادیکال پروستاتکتومی قبلی و رادیاسیون به لگن از مطالعه خارج می گردند. در ابتدا برای کلیه بیماران پرسشنامه استاندارد Beck برای خارج نمودن مبتلایان به اختلالات سایکولوژیک از مطالعه، تکمیل می گردد. سپس شدت اختلال عملکرد نعوظی در شرکت کنندگان، توسط پرسشنامه استاندارد IIEF تعیین می شود. پس از آن بیماران هفته ای یکبار برای شش هفته متوالی، توسط یک اپراتور واحد با استفاده از دستگاه Richard wolf GmbH-Germany(Piezowave2) تحت LI-ESWT قرار می گیرند. دانسیته انرژی مورد استفاده mJ/mm2 160/0 و فرکانس آن HZ8HZ-6 و تعداد کلی 6000 شوک است. در هر بار 2000 موج شوکی به کورپوس کاورنوزوم هر طرف و 1000 موج به کورورای دو طرف اعمال می شود. پس از اتمام درمان از بیماران با مصاحبه حضوری یا تلفنی در مورد عوارض جانبی احتمالی پرسش می شود. همچنین مجددا نمره پرسشنامه IIFF پس از اتمام دوره، سه و شش ماه پس از درمان محاسبه میگردد. آستانه موفقیت درمان به صورت افزایش حداقل 5 نمره در پرسشنامه IIEF درنظرگرفته می شود.

Due to the fact that in the past, the preliminary steps to register a clinical trial in the system started earlier than receiving the ethics approvals, and by taking the approval code, the steps were completed and the proposal was sent, and sometimes it took several months to register in IRCT, so the expected date for starting the study was sometimes much earlier than the due date for it.This project also started about 14 months after the expected date due to financial problems and many improvements in the implementation method. Currently, to prepare the article, it is necessary to modify the dates of start and finish in the system.

Participants/Inclusion and exclusion criteria
Inclusion criteria: age more than 40 years; lack of therapeutic response to phosphodiesterase inhibitors 5.
Exclusion criteria: history of psychological disorders based on the Beck standard questionnaire; history of previous radical prostatectomy; history of pelvic radiotherapy; history of malignancy in the last five years; use of anti-androgens, anticoagulants, thiazides, non-specific antiblockers, opioids and drugs; coronary artery disease; heart failure.
Intervention groups
Intervention group: Low-intensity shock waves (LI-ESWT) with energy density of 0.160 mJ/mm2 and 6HZ-8HZ frequencies and the total number of 6000 shock, once a week for six consecutive weeks

Reason for update
Due to the fact that in the past, the preliminary steps to register a clinical trial in the system started earlier than receiving the ethics approvals, and by taking the approval code, the steps were completed and the proposal was sent, and sometimes it took several months to register in IRCT, so the expected date for starting the study was sometimes much earlier than the due date for it.This project also started about 14 months after the expected date due to financial problems and many improvements in the implementation method. Currently, to prepare the article, it is necessary to modify the dates of start and finish in the system.

Inclusion criteria:
age more than 40 years;
passing more than 6 months from the onset of the disease;
having a stable sex in the last three months;
lack of therapeutic response to phosphodiesterase inhibitors 5;
lack of experience in the use of drugs or non-medical therapies for the treatment of erectile dysfunction in the last three months
Exclusion criteria:
history of psychological disorders based on the Beck standard questionnaire;
history of neurological diseases;
history of previous radical prostatectomy;
history of pelvic radiotherapy;
history of malignancy in the last five years;
use of anti-androgens, anticoagulants, thiazides, non-specific antiblockers, antipsychotics, antidepressants , anxiolytics, opioids and drugs;
alcohol abuse;
severe lower urinary tract symptoms (LUTS);
history of urethritis;
coronary artery disease;
heart failure;
thyroid disorders;
penile or urethra anomalies
