Protocol summary
Objectives: The objective of this study was to assess the affect of treatment with Autologous bone marrow stem cell in autism. Design: This study is a randomized, double blind, case–control study. Setting and conduct: 36 autism patients will be allocated in two groups (18 patients in cases and 18 patients in control group). They will get routine treatment in control group and stem cell transplantation will be done in cases group. Major inclusion and exclusion criteria: children between 5_15 years of age who are met DSM V criteria for diagnosis of Autism Spectrum Disease (ASD), are enrolled and if any serious side effect occurs during treatment or other underlying disease will be excluded.Intervention: After receiving the inform consent from parents, Clinical Global Impression (CGI), The Childhood Autism Rating Scale (CARS), Gilliam Autism Rating Scale (GARS-II) tests will be done. Next, bone marrow aspiration will be done . sample of 50cc of iliac crest will be supplied and will be sent to research center in order to take mesenchymal stem cell . The first intratechal injection will be done.The quality control will be done for the extracted cells And after 4 weeks , mesenchymal cells injection will be done with the method of the first injection . In the third month the tests will be completed. The above named tests will be repeated in 6th and 12th months after starting the treatment .Main outcome measures: The severity of autism criteria will be evaluated during the treatment with these tests.The results will help us to prove the efficacy stem cell injection in the treatment of autism.

مداخلات : بعد دریافت فرم رضایت اگاهانه از والدین تست اظهارات کلی بالینی (CGI) مقیاس درجه بندی اوتیسم کودکی(CARS) و مقیاس درجه بندی اوتیسم کودکی (GARS-II) گرفته خواهد شد. سپس آسپیراسیون مغزاستخوان انجام خواهد شد . نمونهای معادل 50 سی سی از کرست ایلیاک تهیه میشود و جهت تهیه سلول های بنیادی مزانشیمال به پژوهشکده ارسال خواهد شد . اولین تزریق اینتراتکال انجام خواهد شد.کنترل کیفی برای سلول های استخراج شده انجام خواهد شد و بعد از چهارهفته تزریق سلول های مزانشیمال به روش اولین تزریق انجام خواهد شد .درماه سوم تست ها تکمیل خواهد شد.تست های فوق درماه 6و12 بعدشروع درمان مجددا تکرارمی شوند .متغیرهای پیامد اصلی : شدت معیارهای اوتیسم حین درمان با این تست ها ارزیابی خواهدشد .نتایج مطالعه به ما کمک میکند اثر بخشی تزریق سلول بنیادی در درمان اوتیسم اثبات شود.

A long history of allergic reactions, seizures during the past six months, brain trauma, moderate to severe extrapyramidal symptoms and tardive dyskinesia, severe self-injury behavior, autoimmune diseases, severe hematologic and pulmonary disease (such as pneumonia, asthma, malignancy and immune deficiency), severe focal or systemic infection (such as AIDS, hepatitis and syphilis), kidney and liver problems, anemia and hemoglobin level less than 8 g/dL, bone marrow- associated diseases.

The random allocation of patients was done using the numbered, sealed and stapled envelopes. To hide the allocation, random numbers were created using the https://www.random.org/ site and placed by one of the colleagues outside the research team inside the paper envelopes. After explaining the study objectives to the parents of the eligible patients, an envelope was taken for the individual and then, based on the envelope contents, the person was enrolled in the case group receiving SCs and the rehabilitation treatment or in the control group receiving rehabilitation therapy alone

To determine the efficacy and safety of treatment with autologous bone marrow stem cells compared with the routine treatment of children with ASD

The present parallel, single-blind, and randomized clinical trial was conducted on the ASD children referred to the Pediatric Clinic at Ibn-e-Sina Hospital and Establishments covered by the Social Welfare Organization, including the Charity and Rehabilitation Centers and Exceptional Education Centers in Mashhad, Iran

The present parallel, single-blind, and randomized clinical trial was conducted on the ASD children referred to the Pediatric Clinic at Ibn-e-Sina Hospital and Establishments covered by the Social Welfare Organization, including the Charity and Rehabilitation Centers and Exceptional Education Centers in Mashhad, Iran
Participants/Inclusion and exclusion criteria
The inclusion criteria: the age range of ≥5 and ≤15 years and diagnosis of ASD. A long history of allergic reactions, seizures during the past six months, brain trauma, moderate to severe extrapyramidal symptoms and tardive dyskinesia, severe self-injury behavior, autoimmune diseases, severe hematologic and pulmonary disease (such as pneumonia, asthma, malignancy and immune deficiency), severe focal or systemic infection (such as AIDS, hepatitis and syphilis), kidney and liver problems, anemia and hemoglobin level less than 8 g/dL, bone marrow- associated diseases.
Intervention groups
This parallel, single-blind, and randomized clinical trial was conducted on ASD children aged 5-15 years old . The patients were randomly assigned to one of the case group (intervention with MSC plus rehabilitation therapy) or the control group (rehabilitation therapy alone).

Randomization description
The random allocation of patients was done using the numbered, sealed and stapled envelopes. To hide the allocation, random numbers were created using the https://www.random.org/ site and placed by one of the colleagues outside the research team inside the paper envelopes. After explaining the study objectives to the parents of the eligible patients, an envelope was taken for the individual and then, based on the envelope contents, the person was enrolled in the case group receiving SCs and the rehabilitation treatment or in the control group receiving rehabilitation therapy alone
