  • Protocol summary

    Objective: The aim of this study is to determine the effects of a synbiotic food of fortified with beta-carotene on metabolic profiles, inflammatory factor and biomarkers of oxidative stress in patients with diabetic nephropathy. Study design: Randomized double-blinded cross-over controlled clinical trial. Inclusion Criteria: Patients with diabetic nephropathy with FPG ≥ 126 mg/dl, less than 15 years of their disease and aged 35 to 65 years will be included in this study. Exclusion Criteria: The use of substitute treatments including hormone or vitamin supplements, patients with chronic kidney disease, liver, lung and chronic or acute inflammatory diseases, heart valve disease, short bowel syndrome and allergies, patients with low immune system ability (autoimmune) and pregnant or lactating women will be included in this study. Population and sample size: 70 patients with diabetic nephropathy of eligible and referred to Naghavi Clinic affiliated to Kashan University of Medical Sciences, Kashan, Iran in the study will be selected. Intervention: Patients will be assigned to receive either the synbiotic food (intervention group: 35) or control food (control group: 35). Fasting blood samples will be taken at baseline and after 6-wk intervention. A 3-week washout period will be applied. Start and End Date of Intervention: 5 months (January 10, 2013-June 10, 2013). Outcomes: Baseline and End-of-trial fasting plasma glucose (FPG), serum insulin, hs-CRP levels, plasma total antioxidant capacity (TAC) and total glutathione (GSH).

    هدف: هدف این مطالعه تعیین اثرات یک غذای سین بیوتیک غنی شده با بتاکاروتن بر روی پروفایل های متابولیک، فاکتور التهابی و بیومارکرهای استرس اکسیداتیو در بیماران مبتلا به نفروپاتی دیابتیک است. طراحي انجام مطالعه: کارآزمایی بالینی تصادفي دو سوكور متقاطع. معیارهای ورود به مطالعه: بیماران مبتلا به نفروپاتی دیابتیک با FPG ≥ 126 mg/dl، کمتر از 15 سال از سابقه بیماری آن ها و در محدوده سنی 35 تا 65 سال وارد مطالعه خواهند شد. معیارهای خروج از مطالعه: استفاده از درمان های جايگزين از قبیل هورمون يا مكمل هاي ويتاميني، افراد مبتلا به بيماري مزمن كليوي، كبدي، ريوي و بيماري های مزمن يا حاد التهابي، بیماری دریچه قلب، سندروم روده کوتاه و آلرژی، افراد با نقص سیستم ایمنی (اتوایمیون) و زنان باردار و شیرده از مطالعه حذف خواهند شد. حجم نمونه و جمعیت مورد مطالعه: 70 بیمار مبتلا به نفروپاتی دیابتیک واجد شرایط و ارجاع شده به کلینیک نقوی وابسته به دانشگاه علوم پزشکی کاشان، کاشان، ایران در مطالعه انتخاب خواهند شد. مداخله مورد مطالعه: بیماران برای دریافت یا غذای سین بیوتیک (گروه مداخله: 35) یا غذای کنترل (گروه کنترل: 35) تقسیم خواهند شد. نمونه خون ناشتا در ابتدای مطالعه و 6 هفته بعد از مداخله از بیماران گرفته خواهد شد. یک دوره Wash-out سه هفته ای بکار گرفته خواهد. زمان مداخله: 5 ماه (از 21 دی 1391 تا 21 خرداد 1392). پيامدها: گلوکز پلاسمایی ناشتا، سطوح سرمی انسولین، hs-CRP، ظرفیت آنتی اکسیدانی توتال و گلوتاتیون توتال پلاسما در ابتدا و انتهای مداخله اندازه گیری خواهد شد.

Protocol summary

Study aim
Objective: The aim of this study is to determine the effects of a synbiotic food of fortified with beta-carotene on metabolic profiles, inflammatory factor and biomarkers of oxidative stress in patients with type 2 diabetes mellitus.
Design
Study design: Randomized cross-over double-blind controlled trial. Randomization will be done by the use of computer-generated random numbers. Patients will be assigned into two groups to receive synbiotic (n=25) or control (n=26).
Settings and conduct
Among patients with type 2 diabetes mellitus referred to Naghavi outpatient Clinic affiliated to Kashan University of Medical Sciences, Kashan, Iran, 51 patients will be selected according to inclusion and exclusion criteria. Participants, investigators or the assessors of the outcomes are unaware of the study groups. Synbiotic and control are similar in shape and size. Fasting blood samples will be taken at baseline and 6 weeks after the intervention. At the beginning and the end of the intervention: 6 weeks.
Participants/Inclusion and exclusion criteria
Inclusion Criteria: Patients with type 2 diabetes mellitus aged 35 to 70 years. Exclusion Criteria: Pregnant, using insulin or vitamin supplements, liver and inflammatory diseases, coronary heart disease, allergies.
Intervention groups
Intervention group: Probiotic viable and heat-resistance strain Lactobacillus sporogenes (107 CFU), 0.1 g inulin, 0.38 g isomalt, 0.36 g sorbitol, 0.05 g stevia, 0.05 g beta-carotene per 1 g, 9 g, thrice a day for 6 weeks. Control group: 0.38 g isomalt, 0.36 g sorbitol, 0.05 g stevia per 1 g, 9 g, thrice a day for 6 weeks.
Main outcome variables
Outcomes: Insulin (primary outcome), and lipid profiles, biomarkers of inflammation and oxidative stress (secondary outcomes) will be quantified at study baseline and end-of-trial.
