    هدف مطالعه حاضر، ارزیابی اثربخشی مداخله «بازتحکیم القاشده با نشانه و تفکر اپیزودیک آینده» (CIREF) در کاهش ولع مصرف و تنزیل تأخیری در افراد مبتلا به اختلال مصرف مواد افیونی (OUD) تحت درمان نگهدارنده با متادون (MMT) بود.

    کارآزمایی بالینی حاضر دارای گروه کنترل با گروه های موازی، دو سویه کور، تصادفی شده، در فاز ۱ با حجم نمونه ۲۸ است. برای تصادفی سازی از کد های نوشته شده در زبان پایتون استفاده شده است.
    این مطالعه یک کارآزمایی کنترل‌شده پایلوت، چندمرکزی، تصادفی، دوگروهی و با طراحی موازی بود. سی شرکت‌کننده با نسبت ۱:۱ به‌صورت تصادفی به گروه مداخله CIREF (۱۵ نفر) یا گروه کنترل فعال، تفکر اپیزودیک گذشته نزدیک (ERT؛ ۱۵ نفر)، تخصیص یافتند. توالی تصادفی‌سازی با استفاده از Python و روش تصادفی‌سازی بلوکی ایجاد شد. در نهایت، ۲۸ شرکت‌کننده مداخله و ارزیابی پس از مداخله را تکمیل کرده و در تحلیل نهایی وارد شدند (۱۴ نفر در هر گروه).
    This study will be performed in 4 one-hour sessions for four weeks with 28 subjects in one of the outpatient clinics for addiction treatment. After Initial and second screening, eligible individuals will be randomly allocated to trial and control groups. In the first session, momentary craving, craving in the past and delay discounting will be measured. Then, Individuals will be asked to generate opioid-related cues for 6 time intervals and rate each of them. (trial: 1 week, 1, 3, 6 months, and 1 year in future Control: 9,7,5 , Last 3, 1 and 11 days in the past). Cues which will receive normal score in arousal, valence, and vividness will be applied in the intervention. In second sessions, the cues will be presented again and 4 stages of EFT including simulation, prediction, Intention and Plan will be executed. At the end, the amount of craving and the value of procrastination will be measured again.
    This multicentre study was conducted at two outpatient addiction treatment centres in Tehran. After screening, participants were randomized to CIREF or ERT. Ten personalized opioid-related cues were generated and rated, and six were selected. Participants then completed three 75-minute intervention sessions. CIREF used future intervals, whereas ERT used recent-past intervals. DDQ was assessed before and after each session; OCDUS and MCQ were assessed before and after the intervention.
    این مطالعه طی ۴ جلسه یک ساعته‌ به مدت چهار هفته با ۲۸ آزمودنی در یکی از کلینیک‌های درمان سرپائی ترک ‌اعتیاد در استان تهران انجام خواهد شد. پس از انجام غربالگری اولیه و ثانویه افراد واجد شرایط به طور تصادفی در دو گروه مداخله و کنترل جای خواهند گرفت. در جلسه اول مداخله، میزان ولع لحظه ای، ولع در بازه زمانی گذشته، ارزش تعویقی سنجیده خواهد شد. سپس از افراد خواسته می شود تا نشانه های ایجاد کننده ولع در خود را برای ۶ بازه زمانیبازگو کنند و سپس به هر یک از نشانه ها امتیاز دهند (گروه مداخله: ۱هفته، ۱و۳و۶ ماه و ۱ سال آینده گروه کنترل: ۹،۷،۵،۳،۱ و ۱۱ روز گذشته) . نشانه هایی که از لحاظ برانگیختگی، برجستگی و شفافیت نمره کامل را دریافت کرده باشند در جلسات آتی مداخله استفاده خواهند شد. در جلسات مداخله نشانه ها بار دیگر به افراد نشان داده خواهد شد و سپس چهار مرحله مداخله مبتنی بر تفکر آینده نگر رویدادی که شامل شبیه سازی رویداد احتمالی در آینده، پیش بینی دقیق آن رویداد در آينده، هدف جایگزین شده و مراحل انجام هدف است، اجرا خواهد شد. در هر جلسه از مداخله تمرکز بر دو بازه زمانی است. در پایان بار دیگر میزان ولع و ارزش تعویقی سنجیده خواهد شد.
    این مطالعه چندمرکزی در دو مرکز سرپایی درمان اعتیاد در تهران انجام شد. پس از غربالگری، شرکت‌کنندگان به‌صورت تصادفی به گروه CIREF یا ERT تخصیص یافتند. ده نشانه شخصی‌سازی‌شده مرتبط با مواد افیونی تولید و ارزیابی شد و شش نشانه انتخاب شد. سپس سه جلسه مداخله ۷۵ دقیقه‌ای انجام شد. CIREF بر آینده و ERT بر گذشته نزدیک تمرکز داشت. DDQ قبل و بعد از هر جلسه و OCDUS و MCQ قبل و پس از دوره مداخله ارزیابی شدند.

    In the present study, an intervention called " Intervention based on Episodic Future Thinking (EFT)" has been designed for the first time. It consists of 4 one-hour sessions. The protocol tries to be feasible and led participants to remain in the study. The set of opioid narrative cues, applied in the intervention have been completely personalized.
    This pilot study provides the first empirical evaluation of Cue-Induced Reconsolidation and Episodic Future Thinking (CIREF), a manualized intervention integrating personalized drug-cue exposure, memory reactivation, and episodic future thinking. Participants completed one screening and personalized cue-development session followed by three 75-minute intervention sessions. Six individualized opioid-related cues were selected for each participant based on ratings of arousal, salience, valence, and vividness and were used throughout the intervention. The active control condition, Episodic Recent Thinking (ERT), followed a comparable structure while using recent-past rather than future-oriented episodic processing.
    در مطالعه حاضر برای اولین بار مداخله ای با عنوان «مداخله مبتنی بر تفکر آینده نگر رویدادی» طراحی شده است که شامل 4 جلسه یک ساعته می باشد. پروتکل این مداخله بر مبنای انعطاف پذیری نوشته است تا شرکت کنندگان را به ماندن در مداخله و به اتمام رساندن آن سوق دهد. مجموعه نشانه های مواد افیونی به کار گرفته شده در این مطالعه کاملا شخصی‌سازی شده‌اند.
    این مطالعه پایلوت، نخستین ارزیابی تجربی مداخله «بازتحکیم القاشده با نشانه و تفکر اپیزودیک آینده» (CIREF) است؛ مداخله‌ای ساختاریافته که مواجهه با نشانه‌های شخصی‌سازی‌شده مرتبط با مواد، فعال‌سازی مجدد حافظه و تفکر اپیزودیک آینده را تلفیق می‌کند. شرکت‌کنندگان یک جلسه غربالگری و تهیه نشانه‌های شخصی‌سازی‌شده و سپس سه جلسه مداخله ۷۵ دقیقه‌ای را تکمیل کردند. برای هر شرکت‌کننده شش نشانه شخصی‌سازی‌شده مرتبط با مواد بر اساس برانگیختگی، برجستگی، ظرفیت هیجانی و وضوح انتخاب و در مداخله استفاده شد. گروه کنترل فعال ERT ساختاری مشابه داشت، اما پردازش اپیزودیک آن به گذشته نزدیک معطوف بود.
    After screening, participants meeting the required inclusion criteria will be randomly allocated to the intervention and the active control group through block randomization. The randomization list will be created using Python. In block randomization, there would be only six conditions in which two cells get “C” control and two cells get “T” trial for a block of four cells. As a result, the 6 outcomes will be as follows: 1: CCTT, 2: TTCC, 3: CTCT, 4: TCTC, 5: TCCT, 6: CTTC. Also, a number will be randomly assigned to each participant.
    After screening and informed consent, eligible participants were randomly allocated in a 1:1 ratio to the CIREF or ERT group using computer-generated block randomization with a fixed block size of four, applied independently at each recruitment site. Each block contained two CIREF and two ERT assignments, with one of the six possible balanced permutations selected randomly. The allocation sequence was generated in Python by a statistician who was not involved in recruitment, enrollment, or intervention delivery. Allocation concealment was maintained using sequentially numbered assignment codes.

    In the present study, after screening and randomization, individuals will be officially included in the study. In the first session of the intervention, a brief description of the study process by the examiner will be provided to the participant. They will then be given an informed consent form. The information contained in the informed consent form and how to spend the study will be fully explained to the participants. In the process of conducting the study, participants will be blind to the intervention received. It should be noted that the staff present at the outpatient clinic will be blind to the whole intervention process. These people are: nurse, psychologist, assistant and also the technical manager of the clinic. All data recorded during the intervention sessions will be stored in the computer of the outpatient clinic (the venue of the intervention sessions) in an encrypted folder that only the researcher has access to, and will be deleted after the intervention and follow-up sessions. At the end of the intervention sessions, only the data obtained from the questionnaires will be provided to the data analyzer.
    Due to the experiential nature of the interventions—future-oriented episodic simulation in CIREF and recent-past episodic retrieval in ERT—participants could not be blinded to their assigned condition. However, participants were not informed which condition was considered the experimental intervention or active control, and they were not informed of the study hypotheses. The intervention providers were aware of group allocation because this was necessary to deliver the condition-specific procedures. Clinic staff involved in participants’ routine clinical care, including nurses, psychologists, assistants, and the clinic technical manager, remained blinded to group allocation.

    The primary outcome measure of the present study will be momentary craving assessed by the score of 3 sub-scales of the Desire for Drug Questionnaire. The sub-scales including: questions 1, 2, 4, 6, 7, 10, 13, 14 investigate the patient’s “desire and intention to drug use”, questions 5, 9, 11, 12 investigate the “negative reinforcement for drug use" and questions 3 and 8 which are related to “drug abuse control”. There is no normal score for this questionnaire and the significance level of change is the evaluation criterion. Score received in each sub-scales will be assessed at baseline and after intervention.

    Trial group: Episodic Future thinking group: In the first session, the subjects of the intervention group will be asked to generate the opioid-related cues for the the following for the next 6 time intervals, including: 1 day, 1 week, 1 month, 3 months, 6 months and 1 year in future. Subjects will then scored them in terms of Valence, arousal, and vividness.Momentary craving, craving in the past period and the delay discounting will be measured. Next, the subject will be shown the cues of the relevant time intervals on the screen. In the first stage of the Episodic Future thinking, the subject will be asked to simulate a possible future event in his/her mind based on the related cue. In the second stage, he will be asked to predict all the details of the event in 2-3 minutes. The drug-related cues will trigger the drug-related memories which cause in negative retrieval. In the third stage, the subject's memory will be fragile and vulnerable due to stimulation. Now the person will be asked to think about the consequences of substance abuse and form a new interpretation and pursue this interpretation as a goal in his mind. This goal is actually a kind of new memory that is created in a person's mind called reappraisal. In the fourth step, the subject will be asked to plan small but strong steps to achieve the leading plan. These small steps will help him to have a predetermined plan to achieve his goal and also be able to manage his craving toward drug cues. After the intervention sessions, momentary craving, craving in the past period and delay discounting will be measured again.
    Participants in the CIREF group completed three 75-minute intervention sessions following baseline assessment and personalized cue development. Six individualized opioid-related cues were selected for future time intervals (1 day, 1 week, 1 month, 3 months, 6 months, and 1 year). In each session, cue exposure was followed by four stages of episodic future thinking: (1) simulation of a future cue-related event, (2) prediction and appraisal of possible responses and outcomes, (3) formation of a specific adaptive intention or goal, and (4) development of concrete action plans. Phasic craving (DDQ) was assessed before and after each session, while tonic craving (OCDUS) and delay discounting (MCQ) were assessed before and after the intervention period.
    گروه مداخله: گروه تفکر آينده نگر رویدادی: در جلسه اول از آزمودنی‌های گروه مداخله خواسته می‌شود تا برای ۶ بازه‌ی زمانی آینده شامل: ۱ روز آینده، ۱ هفته‌ي آینده، ۱ ماه آینده، ۳ ماه آینده، ۶ ماه آینده و ۱ سال آینده هر کدام نشانه مربوط به ولع مصرف مواد ارائه دهند. سپس این نشانه‌ها از نظر برجستگی، برانگیختگی و شفافیت توسط آزمودنی‌ها نمره‌گذاری می‌شوند. هر نشانه‌ای که نمره‌ي مورد نظر را به دست نیاورد از مطالعه حذف و نشانه‌ی دیگری جایگزین خواهد شد. در ابتدای جلسه اول مداخله ولع لحظه ای، ولع در بازه زمانی گذشته و ارزش تعویقی سنجیده می‌شود. در ادامه، نشانه‌‌‌ی مربوط به بازه‌ی زمانی مربوطه بر روی صفحه‌ي نمایشگر به آزمودنی‌ نشان داده خواهد شد.در اولین مرحله از مداخله‌ی تفکر آینده‌نگر رویدادی از آزمودنی خواسته می‌شود تا بر اساس این نشانه رویدادی احتمالی در آینده را در ذهن خود شبیه‌‌سازی کند. در مرحله‌ي دوم، از او خواسته می‌شود تا تمامی جزئیات مربوط به آن رویداد را در ۲-۳ دقیقه پیش‌بینی کند. نشانه،‌ حافظه‌ي آزمودنی را تحریک کرده و سبب می‌شود تا خاطرات منفی مربوط به سوءمصرف مواد در ذهن او بار دیگر زنده شود. در مرحله‌ي سوم، حافظه‌ی آزمودنی به سبب تحریک شکننده و آسیب پذیر شده است. حال با از فرد خواسته می‌شود تا به پیامد‌های ناشی از سوءمصرف مواد فکر کرده و تعبیر تازه‌ای شکل دهد و این تعبیر را به صورت یک هدف در ذهن خود دنبال کند. این هدف در واقع نوعی از یک خاطره‌ي جدید است که در ذهن فرد ایجاد می شود. در مرحله‌ي چهارم، از آزمودنی خواسته می‌‌‌‌‌شود تا برای رسیدن به هدف پیش‌رو گام‌های کوچک اما محکمی را برنامه‌ریزی کند. این گام‌های کوچک به او کمک خواهد کرد تا برای رسیدن به هدف خود برنامه‌ای از پیش تعیین شده داشته باشد و در صورت مواجهه مجدد با نشانه‌های مواد بتواند ولع خود را مدیریت کرده و دچار لغزش نشود. پس از پایان جلسات مداخله بار دیگر ولع لحظه ای، ولع در بازه زمانی گذشته و ارزش تعویقی سنجیده خواهد شد.

    Participants in the ERT active control group completed the same intervention dose, session structure, cue-exposure procedures, and therapist-guided exercises as the CIREF group. Six individualized opioid-related cues were linked to recent-past intervals (1, 2, 3, 5, 7, and 9 days earlier). Each cue was followed by episodic retrieval of a recent-past event, prediction and appraisal of responses and outcomes, intention formation, and planning. The main difference from CIREF was temporal orientation: past-oriented retrieval in ERT versus future-oriented simulation in CIREF. DDQ was assessed before and after each session, while OCDUS and MCQ were assessed before and after the intervention period.

Study aim
The current study aimed to evaluate the efficacy of Cue-Induced Reconsolidation and Episodic Future Thinking (CIREF) in reducing craving and delay discounting in individuals with opioid use disorder (OUD) receiving methadone maintenance treatment (MMT).

The current study aimed to evaluate the efficacy of Cue-Induced Reconsolidation and Episodic Future Thinking (CIREF) in reducing craving and delay discounting in individuals with opioid use disorder (OUD) receiving methadone maintenance treatment (MMT).
Settings and conduct
This multicentre study was conducted at two outpatient addiction treatment centres in Tehran. After screening, participants were randomized to CIREF or ERT. Ten personalized opioid-related cues were generated and rated, and six were selected. Participants then completed three 75-minute intervention sessions. CIREF used future intervals, whereas ERT used recent-past intervals. DDQ was assessed before and after each session; OCDUS and MCQ were assessed before and after the intervention.
Participants/Inclusion and exclusion criteria
Inclusion Criteria: Individuals with a history of opioid use currently receiving Methadone maintenance treatment Acquire normal score on the Beck Depression Inventory, Snaith-Hamilton Pleasure Scale and Montreal Cognitive Assessment in the initial Screening Ability to generate the opioid-related cues in the second screening Exclusion Criteria: Not having the mentioned criteria above

After screening and informed consent, eligible participants were randomly allocated in a 1:1 ratio to the CIREF or ERT group using computer-generated block randomization with a fixed block size of four, applied independently at each recruitment site. Each block contained two CIREF and two ERT assignments, with one of the six possible balanced permutations selected randomly. The allocation sequence was generated in Python by a statistician who was not involved in recruitment, enrollment, or intervention delivery. Allocation concealment was maintained using sequentially numbered assignment codes.

Due to the experiential nature of the interventions—future-oriented episodic simulation in CIREF and recent-past episodic retrieval in ERT—participants could not be blinded to their assigned condition. However, participants were not informed which condition was considered the experimental intervention or active control, and they were not informed of the study hypotheses. The intervention providers were aware of group allocation because this was necessary to deliver the condition-specific procedures. Clinic staff involved in participants’ routine clinical care, including nurses, psychologists, assistants, and the clinic technical manager, remained blinded to group allocation.

This pilot study provides the first empirical evaluation of Cue-Induced Reconsolidation and Episodic Future Thinking (CIREF), a manualized intervention integrating personalized drug-cue exposure, memory reactivation, and episodic future thinking. Participants completed one screening and personalized cue-development session followed by three 75-minute intervention sessions. Six individualized opioid-related cues were selected for each participant based on ratings of arousal, salience, valence, and vividness and were used throughout the intervention. The active control condition, Episodic Recent Thinking (ERT), followed a comparable structure while using recent-past rather than future-oriented episodic processing.

Primary outcomes

Description
The primary outcome measure of the present study will be momentary craving assessed by the score of 3 sub-scales of the Desire for Drug Questionnaire. The sub-scales including: questions 1, 2, 4, 6, 7, 10, 13 investigate the patient’s “desire and intention to drug use”, questions 5, 9, 11, 12 investigate the “negative reinforcement for drug use" and questions 3 and 8 which are related to “drug abuse control”. There is no normal score for this questionnaire and the significance level of change is the evaluation criterion. Score received in each sub-scales will be assessed at baseline and after intervention.

Description
14The primary outcome measure of the present study will be craving in the past period assessed by the score of 4 sub-scales of Obsessive Compulsive Drug Use Scale. The sub-scales including: questions 1, 2, 7, 8 investigate “desire and mental preoccupation with drugs”, questions 3 and 9 could be called as “the effect of desire for drug, and drug-related thoughts on the patient’s work and life”, questions 4, 6, 11 and 12 are related to “motivation, emotion, and lack of control” and questions 5 and 10 evaluate “resistance to drug use”. score for this questionnaire and the significance level of change is the evaluation criterion. Score received in each sub-scales will be assessed at baseline and after intervention.

Secondary outcomes

Description
The secondary outcome of the present study is the score obtained in the dual monetary selection questionnaire.

Delay Discounting will be assessed by Monetary Choice Questionnaire.Monetary Choice Questionnaires is a 27-item, brief dichotomous choice tasks that assess preference between small immediate and larger delayed monetary outcomes.

Intervention groups

Description
Participants in the CIREF group completed three 75-minute intervention sessions following baseline assessment and personalized cue development. Six individualized opioid-related cues were selected for future time intervals (1 day, 1 week, 1 month, 3 months, 6 months, and 1 year). In each session, cue exposure was followed by four stages of episodic future thinking: (1) simulation of a future cue-related event, (2) prediction and appraisal of possible responses and outcomes, (3) formation of a specific adaptive intention or goal, and (4) development of concrete action plans. Phasic craving (DDQ) was assessed before and after each session, while tonic craving (OCDUS) and delay discounting (MCQ) were assessed before and after the intervention period.

Description
Participants in the ERT active control group completed the same intervention dose, session structure, cue-exposure procedures, and therapist-guided exercises as the CIREF group. Six individualized opioid-related cues were linked to recent-past intervals (1, 2, 3, 5, 7, and 9 days earlier). Each cue was followed by episodic retrieval of a recent-past event, prediction and appraisal of responses and outcomes, intention formation, and planning. The main difference from CIREF was temporal orientation: past-oriented retrieval in ERT versus future-oriented simulation in CIREF. DDQ was assessed before and after each session, while OCDUS and MCQ were assessed before and after the intervention period.

Title and more details about the data/document
De-identified participant-level dataset and supporting analysis materials for the CIREF pilot randomized controlled trial. Details: De-identified participant-level data used in the reported analyses may be shared upon reasonable request, including demographic and clinical variables, cue ratings, treatment-group assignment, DDQ, OCDUS, and MCQ outcomes, together with relevant derived variables, a data dictionary, and statistical analysis code. Direct identifiers and potentially identifying free-text information, including personalized cue content, will not be shared.

The de-identified participant-level data and supporting materials will be available upon reasonable request after publication of the primary study results. There is no predetermined end date for data availability; requests may be submitted for as long as the study team is able to maintain and securely share the data.

De-identified participant-level data and supporting documents may be shared, upon reasonable request, with qualified researchers from academic, clinical, governmental, or other legitimate research institutions. Requests from researchers outside academia may also be considered if the proposed use is scientifically appropriate and consistent with ethical and confidentiality requirements.

De-identified participant-level data and supporting materials may be used for scientifically appropriate secondary analyses, replication and reproducibility studies, meta-analyses, methodological research, or related academic purposes.
