The study will be in academic hospitals or private offices.Patient first evaluated by main resarcher(who is a cardiologist)for enterence criteria.Then randomuzation will be by a external technician using computet program.The concealed enveope of the randomization result will be open by main researcher and true or sham intervetion eill be done.Bp will be measured by personel and questionary filled by patients.The patients will be educated regarding trial.

بیماران از بیمارستانهای دانشگاهی یا مطب ها ابتدا توسط محقق اصلی که خود متخصص قلب است ارزیابی میشوند تا شرایط ورود به مطالعه را داشته باشند.سپس توسط فردی خارج از تیم مطالعه توسط کامپیوتر بطور تصادفی به دو گروه اکوپرشر واقعی و شم قرار می گیرند و نایجه با پاکت دربسته تحویل محقق میشود .بیماران توسط پرسنل فشارخون اندازه گیری شده و پرسشنامه پر میشود.بعد از چسباندن و اموزش لازم و توضیح مطالعه بیماران محددا فشار توسط پرسنل کنترل میشود.پرسنل و جمع کننده داده ها و انالیز کننده و بیمار کور است
قیل از مطالعه هدف و نحوه انجام مطالعه به افراد توضیح داده شد و یک رضایت نامه کتبی برای شرکت در مطالعه از شرکت کنندگان گرفته شد.افراد این مطالعه از مراکز بهداشتی دانشگاه علوم پزشکی مشهد در شهر مشهد در ایران بودند.معیارهای ورود بزرگسالان مساوی یا بالالر از 18 سال با فشارخون اساسی بودند که در طی یکماه اخیر فشار زیر 180/120 داشتند و طب سوزنی نمی گرفتند و در مالعه دیگری شرکت نداشتند.معیارهای خروج داشتن عوارض قلبی عروقی مثل نارسایی قلبی یا سکته مغزی .سایر بیماریهای قابل توجه مثل بیماری کلیوی یا کبدی .مصرف داروی ضد صرع و تشنج و زنان باردار و شیرده

معیارهای ورودو عدم ورود افراد به مطالعه:۱.سن بالای ۱۸ سال۲.فشارخون ثانویه نباشد.۳.در یکماه اخیر فشارخون زیر ۱2۰/۱۸۰میلی مترجیوه باشد.۴.حامله یا شیرده نباشد.۵.عوارض قلبی عروقی نداشته باشد.۶.بیماریهای همراه مثل بیماری کلیه ،ریه ،کبد،خودایمنی و بدخیمی۷.داروهای دیگری مانند ضد انعقاد و ضد تشنج مصرف ننماید.۸.قادر به ماساژ گوش باشد.۹.در کارازمایی دیگری وارد نشده باشد.۱۰.اکوکاردیوگرافی و نوارقلب اخیر و ازمايش خوني معمول داشته باشد.۱۱.نداشتن سابقه استفاده از طب فشاری گوش

طب فشاری گوش حقیقی و شم (ساختگی)
گروه مداخله یک شامل بیماران با فشارخون اساسی می باشند که مداخله چسباندن برچسب حاوی سوزن را دریافت می کنند.
گروه مقایسه یک شامل بیماران با فشارخون اساسی می باشند که مداخله چسباندن برچسب بدون سوزن را دریافت می کنند.
طب فشاری گوش حقیقی و شم (ساختگی)گروه مداخله یک شامل بیماران با فشارخون اساسی می باشند که مداخله چسباندن برچسب حاوی سوزن را دریافت می کنند. گروه مقایسه یک شامل بیماران با فشارخون اساسی می باشند که مداخله چسباندن برچسب بدون سوزن را دریافت می کنند.

روش پنهان سازی بدین صورت است که فرد مسوول تصادفی سازی خارج از تیم پژوهشی بعد از تصادفی سازی کامپیوتری نتیجه را در پاکت مهر وموم شده به بیمار می دهد و فرد مسوول انجام مداخله براساس ان چسبهای حاوی سید یا فاقد ان(شم)را برای بیمار می چسباند
این یک تصادفی سازی ساده است.هر بیمار یک پاکت سربسته حاوی برگه آ یا ب برای گروه واقعی یا شم برمی دارد.این پاکتها توسط محقق امار مطالعه تهیه شده است.محقق اصلی براساس برگه آ یا ب برچسب حاوی سوزن یا بدون سوزن را به بیماران می چسباند.این برچسب ها از نظر ظاهری مثل هم هستند.

بیماران در مرحله اول کور هستند و نمیدانند درکدام گروه هستند.پرسنل بهداشتی که وظیفه کنترل فشارخون و ثبت داده ها را دارند هم اطلاعی از گروه بیماران(شم یا واقعی)ندارند.محقق اصلی ولی چون تگ ها را میچسباند از گروه بیماران اطلاع دارد .ولی محقق امار و انالیز کننده هم نسبت به گروه ها کور است.
بیماران و کادر بهداشتی اندازه گیرنده فشار خون و محقق انالیز کننده و ارزیاب و جمع کننده نتایج نسبت به گروه بندی کور هستند ولی محقق اصلی که برچسب ها را می چسباند کور نیست.

Control group: sham(without seed )plaster at acupoints on ear
After randomization,standard SF-36questionarie filled out by patients and blood pressure measured by digital arm sphygmomannometer.4 adhesive tape containg1mm needle in its center are pasted to specific ear points(shenmen,superior triangular fossa,Chinese liver and heart )on right ear.7 days later patients will come back and tapes removed and blood pressure be measured and new tapes pasted on the points on left ear.4 times (4weeks) will be dine again with tapes 2weeks on right and 2weeks on left ear.at end of last session SF-36questionarie will be filled by patients.after one month patients will come back for follow up and blood pressure will be measured and the questionnaire will be filled.

بعد از گروه بندی تصادفی پرسشنامه استاندارد sf-36توسط بیماران پر شده و فشار خون انهابا فشارسنج دیجیتالی بازویی اندازه گیری میشود.4برچسب حاوی سوزن 1میلی متری در وسط ان به نقاط خاص گوش راست (شن من .فوقانی حفره مثلثی.کبد و قلب چینی)چسبانده میشوند.7روز بعد بیماران بر می گردند و برچسب انها برداشته شده و فشارخون شان اندازه گیری میشود و برچسب ها در نقاط مخصوص در گوش چپ چسبانده میشود..4 بار (4هفته)اینکار انجام میشود.دوهفته گوش راست و دو هفته گوش چپ.در انتهای جلسه اخر پرسشنامه sf-36مجددا توسط بیماران پر میشود.بعد از یکماه بیماران برای پیگیری مراجعه می کنند و فشارخونشان اندازه گیری شده و پرسشنامه پر میشود.
بعد از گروه کنترل:بندی تصادفی پرسشنامه استاندارد sf-36توسط بیماران پر شده و فشار خون انهابا فشارسنج دیجیتالی بازویی اندازه گیری میشود.4برچسب حاوی سوزن 1میلی متری در وسط ان به نقاط خاص گوش راست (شن من .فوقانی حفره مثلثی.کبد و قلب چینی)چسبانده میشوند.7روز بعد بیماران بر می گردند و برچسب بدون سیدانها برداشته شده و فشارخون شان اندازه گیری میشود و برچسب ها در مکانهای اکوپرشرنقاط مخصوص در گوش چپ چسبانده میشود..4 بار (4هفته)اینکار انجام میشود.دوهفته گوش راست و دو هفته گوش چپ.در انتهای جلسه اخر پرسشنامه sf-36مجددا توسط بیماران پر میشود.بعد از یکماه بیماران برای پیگیری مراجعه می کنند و فشارخونشان اندازه گیری شده و پرسشنامه پر میشود.

Study aim
Measurement of changes in systolic and diastolic blood pressure before and after intervention and after 1months followup after ear acupuncture intervention
Design
Randomized, sham-controlled , parallel group trial with blinded outcome assessment. Randomization was centralized and computerized with concealed randomization sequence carried out by an external technician.sample volume was60 and each group had 30 patients.
Settings and conduct
.Before conducting the study, the purpose and procedure of the study were explained

The subjects of this study were recruited from Mashhad university of medical science health centers in Mashhad, Iran. The inclusion criteria were adult people over 18 years old with
essential hypertension,who had not have blood pressure more than 180/120mmhg in preceding month , and not taking acupuncture or participating in another study. The exclusion criteria were those who had cardiovascular complication such as heart failure or stoke,other significant diseases such as liver or renal disease,taking drugs for seizure or cancer,women who were pregnant or breastfeeding,
Participants/Inclusion and exclusion criteria
Inclusion criteria :1.age>=18. 2.Essential hypertension(not secondary) 3..BP<180\120 in preceding month
Exclusion criteria:1. cardiovascular complications 2.accompanying diseases such as kidney,lung,liver,autoimmune diseases and cancer 3. taking other drugs such as anticoagulant or seizure drugs.
Intervention groups
intervention group are patients with essential hypertension who receive. needle- containing adhesive tapes .
placebo group are patients with essential hypertension who receive. needle- free adhesive tapes .
change in method which includes substitution of needle containing tages ,so no need to pressure on tages because needle stimulates the points and in this method patients will be completely blinded.

This is simple randomization .Each patient takes an concealed envelope containing letter A or B for true or sham group.These envelopes were provided by statistic analyzer of the study.
The main researcher pastes either true needle tag or sham needle-free tag to the patients based on letter A or B.These tags are apparently identical.
