Determining the effect of video-based stroke triage training on triage decision-making of nurses in the emergency department
Design
A field trial with a pretest-posttest design, including 54 eligible nurses who were conveniently recruited and allocated using block randomization
Settings and conduct
This study includes nurses in the emergency department of Shohada Ashayer hospital in Khorramabad in 2025. The groups will be randomly assigned by an independent nurse, and coded as A (intervention) and B (control Groups will be randomly assigned with the codes remaining confidential until the end of data analysis. The data analyst, evaluating nurse (data collector), and study participants will be blinded to group allocation. However, due to the nature of the study, blinding of the intervention providers will not be possible.
Participants/Inclusion and exclusion criteria
Nurses working in the emergency department; nurses with at least one year of work experience or more; willingness to participate in a training program; access to the internet; access to a computer or mobile phone; nurses with Bachelor of Science in Nursing degree (BSN) or higher.
Intervention groups
First, 7 video-based scenarios will be designed based on a review of the literature and the Iranian stroke triage scale of Abedi et al.
The total intervention time is 4 hours. The nurses in the intervention group will be divided into three groups, and each group will be trained based on the content of the scenarios (training with the show of designed videos) in two sessions and each session lasting 2 h. Then, their triage decision-making will then be measured by a questionnaire after two weeks of the intervention.

Randomization description
54 eligible emergency department nurses will be conveniently recruited into the study. It using a four-block randomization method to assign nurses to two group. After obtaining written informed consent from the participants, 6 different combinations of four-block (AABB- ABAB- ABBA- BBAA- BABA- BAAB) were created. It randomly selected using the Blockrand package of R software, and the random sequence will be formed as a sequence of letters A and B.

Description
Intervention group: First, 7 video-based scenarios will be designed based on a review of the literature and the Iranian stroke triage scale of Abedi et al. Scenarios content includes, how to triage unconscious stroke patients with unstable vital signs, triage of semi-conscious stroke patients with a history of hypertension, diabetes, hemiplegia weakness, and neurological symptoms for more or less than one hour, and triage of conscious stroke patients with normal blood pressure without dysphasia and unilateral body weakness with neurological symptoms for less than 10 minutes. The total intervention time is 4 hours. The nurses in the intervention group will be divided into three groups, and each group will be trained based on the content of the scenarios (training with the show of designed videos) in two sessions and each session lasting 2 h.
