Objective:
Comparison of two different doses of thiopental sodium in elective cesarean section (C/S) under general anesthesia on depth of anesthesia with Isolated Forearm Technique (IFT) and on neonatal neurobehavioral test
Inclusion criteria: pregnant women (ASA I- II) who are under general anesthesia for elective cesarean section.
Exclusion criteria:
Patients who are not properly cooperate; prior use of sulfate MgSO4; mental disorders; a history of previous awareness during general anesthesia, taking blood pressure medication or vasodilators or vasodepressor and human stimulant or psychotropic drugs; neuromuscular diseases, opium addicts; detection of any fetal abnormalities by ultrasonography.
Intervention:
70 patients divided into two groups one or two: Anesthesia will induce by Thiopental (5mg / kg) in group one and Thiopental (7 mg / kg) in group two and Scholin (2 mg / kg) , maintenance was with 50% N2O, O2, Sevoflurane
and after birth by N2O 50%, O2, sevoflurane blood pressure cuff will fasten on the patients’ arm and inflated (200 mmHg) immediately before administering the drugs.
After induction, a recorded message will be played once by the headphones every 30 seconds to move the fingers of the dominant hand. Hand movement will be scored as follows after playing message: No movement: zero /uncertain movements: open and close the fingers: 2 before induction of anesthesia, BIS sensor device will be installed according to the manufacturer's instructions on the forehead of the patient. BIS values and IFT following events will be recorded at baseline,
Induction of anesthesia, laryngoscopy and intubation, Skin incision, opening the peritoneum, Uterine incision, uterine retraction, childbirth, Uterine closure, tying the layers under the skin, Subcutaneous closures, at the start of skin closure,2 minutes after skin closure, eye opening and extubation. The neonate Apgar score is measured at 0, 5, and 20 minutes after birth and neurobehavioral test evaluates after the birth of the newborn.

شرایط ورود به مطالعه: خانم های باردار ؛( ASA I- II) که جهت عمل سزارین انتخابی تحت بیهوشی عمومی قرار می گیرند. شرایط خروج از مطالعه: عدم همکاری مناسب بیمار؛ دریافت داروی MgSO4 قبل از انجام مطالعه ؛ اختلالات روانی ؛ سابقه awareness در بیهوشی های قبلی ؛ مصرف داروهای (ضدفشار خون ،اعصاب یا داروهای رونگردان , وازوپرسور یا وازودیلاتور )؛ سابقه بیماری های عصبی-عضلانی ؛ افراد معتاد به مواد مخدر؛ هر نوع سونوگرافی جنینی غیرنرمال.
مداخلات:70 بیمار در دو گروه یک و دو تقسیم می شوند القاء بیهوشی با تجویز 5 میلی گرم بر کیلو گرم تیوپنتال سدیم در گروه اول و 7میلی گرم بر کیلو گرم تیوپنتال سدیم در گروه دوم و2 میلی گرم بر کیلو گرم سوکسینیل کولین و نگهداری بیهوشی بوسیله اکسیژن 50% و N2O 50% و سووفلوران و پس از تولد نوزاد بوسیله سووفلوران و اکسیژن 50% و N2O 50% خواهد بود.
یک عدد کاف دستگاه فشاخون بر روی بازوی بیمار بسته می شود. و بلافاصله قبل از تجویز داروها به میزان 200 mmHgباد می شود. پس از القاء بیهوشی هر 30 ثانیه یک بار پیغام ضبط شده ای توسط هدفون برای بیمار پخش می شود. فعالیت دست پس از پخش پیام به صورت زیر امتیازبندی خواهد شد: بدون حرکت: صفرحرکات نامشخص: 1باز و بسته کردن انگشتان: 2قبل از القاء بیهوشی sensor دستگاه BIS بر اساس دستورالعمل شرکت سازنده بر روی پیشانی بیمار نصب خواهد شد.
مقادیر BISو IFT در هنگام هر یک از وقایع زیر: مقدار پایه، القاء بیهوشی، لارنگوسکوپی، انتوباسیون، انسزیون پوست، باز کردن پریتونئوم، انسزیون رحم، رترکشن رحم، تولد نوزاد،بستن رحم،بستن لا یه های زیر پوست ،بستن زیر پوست،شروع بستن پوست،دو دقیقه بعد از بستن پوست، بازکردن چشمها، اکستوباسیون. آپگارنوزاد در دقایق صفر و پنج و بیست و neurobehavioral test بیست دقیقه بعد از تولد نوزاد بررسی می شوند.

An anesthesia nurse is providing the Thiopental syringe to the anesthesiologist for induction of anesthesia .The anesthesiologist who are responsible for injecting Thiopental is aware of the study groups while the another anesthesiologist who will assess the mothers and infants during the study is unaware of the study groups .

Study aim
Objective:Comparison of two different doses of thiopental sodium in elective cesarean section (C/S) under general anesthesia on depth of anesthesia with Isolated Forearm Technique (IFT) and on neonatal neurobehavioral test
Design
Double blind randomized clinical trial
Settings and conduct
patients divided into two groups one or two in the Hafeiz Hospital .Induction of anesthesia will be with Thiopental (5mg / kg) in group one and Thiopental (7 mg / kg) in group two and Scholin (2 mg / kg) , maintenance was with 50% N2O, 50%O2, Sevoflurane and after birth by N2O 50%, O2 50%, sevoflurane.
Participants/Inclusion and exclusion criteria
Inclusion criteria: pregnant women (ASA I- II) who are under general anesthesia for elective cesarean section.Exclusion criteria: patients who are not properly cooperate; prior use of sulfate MgSO4; mental disorders; a history of previous awareness during general anesthesia, taking blood pressure medication or vasodilators or vasodepressor and human stimulant or psychotropic drugs; neuromuscular diseases, opium addicts; detection of any fetal abnormalities by ultrasonography.
Intervention groups
80 patients divided into two groups : Induction of anesthesia will be with Thiopental (5mg / kg) in group one and Thiopental (7 mg / kg) in group two and Scholin (2 mg / kg) , maintenance was with 50% N2O,50%O2, Sevoflurane
and after birth by N2O 50%, 50%, sevoflurane.

An anesthesia nurse is providing the Thiopental syringe to the anesthesiologist for induction of anesthesia .The anesthesiologist who are responsible for injecting Thiopental is aware of the study groups while the another anesthesiologist who will assess the mothers and infants during the study is unaware of the study groups .

To determine the depth of the anesthesia by Sodium Thiopental and Propofol as an induction anesthetic
