Protocol summary
The objective of this study is evaluating the effect of delayed start protocol in ovarian poor responders. Study population: Infertile women with poor ovarian response candidate for IVF. Study groups: Intervention and control groups. There are thirty women in intervention group and thirty in control group. This is a randomized not blinded trail, in one center, in phase 2 study. Setting and conduct: Ovarian poor responders after obtaining informed consent randomized into intervention and control groups. Inclusion criteria: Patient with Bologna criteria for ovarian poor responders.
Exclusion criteria: History of endocrine disorders, severe endometriosis, azoospermia. Control group treated with conventional antagonist protocol and intervention group with delayed protocol. Pretreatment with 4mg estradiol valerate was administered in both groups. In the delayed-start protocol, ovarian stimulation begins after 7 days of GnRH antagonist pretreatment (0.25 mg cetrorelix acetate). Ovarian stimulation with 375 IU FSH administered for both groups.
GnRH antagonist will prevent premature ovulation when the lead follicle measured 12 mm and continued until the hCG trigger. Final oocyte maturation will trigger with 10,000 IU hCG. Oocyte retrieval performs under transvaginal ultrasound guidance 34-36 hours after hCG triggering. Intracytoplasmic sperm injection (ICSI) performed with mature oocytes (metaphase II [MII]) in all cycles. Embryos will transfer 2 or 3 days after oocyte retrieval. Primary outcomes: Number of mature oocytes(MII) , total number of oocytes. Secondary outcomes: Number of embryo, fertilization rate, implantation rate, chemical and clinical pregnancy.

هدف از این مطالعه ارزیابی تاثیر پروتکل تاخیری در درمان بیماران با پاسخ ضعیف تخمدان می باشد. جمعیت مورد مطالعه بیماران نابارور با پاسخ ضعیف تخمدان به تحریک که کاندید لقاح ازمایشگاهی هستند می باشد. گروه های مطالعه: دو گروه مداخله و کنترل. حجم نمونه 60 بیمار، کور نشده، تصادفی شده به دو گروه و یک مطالعه تک مرکزی و فاز 2. نحوه انجام: بیماران ناباروری که پاسخ دهنده ضعیف تخمدان به تحریک باشند وارد مطالعه شده و پس از دادن رضایت نامه اگاهانه به صورت تصادفی در دو گروه قرار می گیرند. شرایط ورود: بیمارانی که معیارهای بولونیا برای تعریف پاسخ ضعیف تخمدان را داشته باشند. شرایط عدم ورود: بیماران با سابقه بیماریهای اندوکرین؛ بیماران با سابقه اندومتریوز شدید؛ بیماران ازواسپرمی. مداخلات: در گروه کنترل پروتکل معمول انتاگونیست و در گروه مداخله پروتکل تاخیری شروع خواهد شد. در هر دو پروتکل بیماران از روز 20 سیکل قاعدگی قبلی قرص استرادیول والرات 4 میلی گرم روزانه به مدت 10 روز دریافت خواهند کرد. در گروه مداخله، تحریک تخمدان پس از 7 روز تجویز روزانه ی آنتاگونیست هورمون ازاد کننده گونادوتروپین، 0.25 میلی گرم سترورلیکس، شروع می شود. در هر دو گروه تحریک تخمدان با 375 واحد FSH شروع شده و زمانی که فولیکول پیشرو به حداقل سایز 12 میلی متر رسید، مجددا آنتاگونیست GnRh جهت پیشگیری از اوولاسیون زودرس شروع شده و تا زمان تریگرینگ بلوغ نهایی اووسیت با (HCG 10000 واحد) ادامه داده خواهد شد. در هر دو گروه 34 تا 36 ساعت بعد از تزریق HCG، پونکسیون انجام خواهد شد و ICSI بر روی اووسیتهای متافاز دو صورت خواهد گرفت. پس از تشکیل جنین انتقال در روز 2 یا 3 پس از پونکسیون انجام خواهد شد.
پیامدهای اولیه: تعداد اووسیت بالغ ، تعداد کل اووسیت. پیامدهای ثانویه: تعداد جنین، میزان لقاح، میزان لانه گزینی، حاملگی بیوشیمیایی و کلینیکی

Poor ovarian responders according to Bologna criteria. At least two of the following three features must be present:
Advanced maternal age (≥40 years) or any other risk factor for poor ovarian response;
A previous poor ovarian response (≤3 oocytes with a conventional stimulation protocol);
An abnormal ovarian reserve test (i.e. AFC <5–7 follicles or AMH <1.1 ng/ml).
Two episodes of POR after maximal stimulation are sufficient to define a patient as poor responder.

Study aim
effect of delayed start protocol in ovarian poor responders
Design
InterventionalClinical trial with control group, non-blind, randomized, phase 2 on 60 patients. Closed envelope randomization was used for randomization.
Settings and conduct
The objective of this study is evaluating the effect of delayed start protocol in ovarian poor responders. Study population: Infertile women with poor ovarian response candidate for IVF. Study groups: Intervention and control groups. There are thirty women in intervention group and thirty in control group. This is a randomized not blinded trail, in one center, in phase 2 study. Setting and conduct: Ovarian poor responders after obtaining informed consent randomized into intervention and control groups.
Participants/Inclusion and exclusion criteria
Inclusion criteria: Patient with Bologna criteria for ovarian poor responders
Exclusion criteria: History of endocrine disorders, severe endometriosis, azoospermia.
Intervention groups
In the intervention group with delayed protocol: First, like the control group, estradiol valerate 4 mg daily for 10 days starts from the 20th day of the previous menstrual cycle, then 7 days of daily administration of gonadotropin-releasing hormone antagonist (0.25 mg cetrorelix acetate) begins. The ovaries are then stimulated with gonadotropins.
Main outcome variables
Number of mature oocytes(MII) , total number of oocytes, Number of embryo, fertilization rate, implantation rate, chemical and clinical pregnancy.

Inclusion criteria:
Poor ovarian responders according to Bologna criteria. At least two of the following three features must be present:
Advanced maternal age (≥40 years) or any other risk factor for poor ovarian response;
A previous poor ovarian response (≤3 oocytes with a conventional stimulation protocol);
An abnormal ovarian reserve test (i.e. AFC <5–7 follicles or AMH <1.1 ng/ml).
Two episodes of POR after maximal stimulation are sufficient to define a patient as poor responder.
