    Introduction: Coronary artery disease is the most important disorder of the cardiovascular. Coronary artery bypass graft surgery (CABG) is a common and successful method of treatment of coronary artery disease. Brain injury occurs after cardiac surgery. A more common form of brain injury is cognitive deficits such as deterioration in memory, attention, (psycho)motor speed, and visuo-spatial ability. In fact, memory and attention deficits are frequent after (CABG) surgery. Furthermore, cognitive deficits are associated with decreased of quality of life. In broad terms, rehabilitation principally focuses on the enhancement of human functioning and quality of life. Cognitive rehabilitation includes remediation and training and is used to rehabilitate thinking skills (e.g., attention, memory) impaired. Objectives: The aim of this study was to enhance the quality of life in patients undergoing CABG by using a computerized attention training program to improve cognitive deficits. Method: In this semi experimental study, a randomized controlled trial with pre-test, post-test, and follow-up assessments along with placebo and control groups was conducted. In this clinical trial, 75 patients undergoing coronary artery bypass graft surgery selected by convenience sampling among patients who are undergoing rehabilitation at Tehran Heart Center. They randomly assigned to experimental, placebo and control groups. The participants complete assessments including the test of Continuous Performance, by the test of Flanker, the test of useful Field Of view, the test of Digit Span and a Quality of Life questionnaire (SF-36) at three time points: pre and post intervention as well as on-month follow-up.

    مقدمه: بیماری عروق کرونر مهم‌ترین اختلال بیماری‌های قلبی-عروقی است. جراحی بای‌پس عروق کرونر یک روش رایج و موفق در درمان بیماری عروق کرونر است. آسیب‌های مغزی پس از جراحی‌های قلبی اتفاق می‌افتد. شکل رایج تر آسیب مغزی، نقص‌های شناختی مانند زوال حافظه، توجه، سرعت روانی حرکتی، و توانایی دیداری فضایی است. در حقیقت نقص‌های حافظه و توجه پس از جراحی بای‌ عروق کرونر متدوال است. همچنین، نقص‌های شناختی با کاهش کیفیت زندگی همراه است. در یک اصطلاح عام، بازتوانی عمدتا بر افزایش عملکرد انسان و کیفیت زندگی متمرکز است. بازتوانی شناختی شامل درمان و آموزش است و جهت بازتوانی نقص مهارت‌های فکری ( مثل؛ توجه، حافظه) از آن استفاده می‌شود. اهداف: این پژوهش به منظور افزایش توانمندی‌های مغزی (توجه و حافظه) و در نهایت ارتقاء کیفیت زندگی بیماران تحت جراحی بای‌پس عروق کرونر با بکارگیری تمرینات کامپیوتری مغزینه انجام می‌گیرد. روش: در این مطالعه نیمه تجربی، یک کارآزمایی کنترل شده تصادفی با ارزیابی‌های پیش آزمون، پس‌آزمون و پیگیری همراه با گروه کنترل و دارونما انجام می‌شود. در این کارآزمایی بالینی، 75 بیمار تحت عمل جراحی CABG از طریق روش نمونه‌گیری دردسترس از میان بیماران تحت بازتوانی در مرکز قلب تهران انتخاب می‌شوند. سپس به صورت تصادفی در سه گروه آزمایش، پلاسیبو و کنترل قرار می‌گیرند. شرکت‌کنندگان ابزار سنجش عملکرد مغزی شامل فراخنای ارقام، آزمون عملکرد مدام، فلنکر و Useful Field of View و پرسشنامه‌ی کیفیت زندگی را در سه مرحله‌ی پیش‌آزمون، پس‌آزمون و یک ماه پیگیری تکمیل می‌کنند.

    Experimental: cognitive intervention This group of patients will be under intervention by computerized trainings of the Maghzineh Package to increase brain activation. This package has 24 exercise steps that the difficulty of exercises gradually increases in each step. These steps are carried out during the three months (two days a week) for each patient.
    Experimental Group:This group of patients received CCRT plus RCR to activate different cognitive functions including attention, working memory and inhibition. This package has 24 exercise steps that the difficulty of exercises gradually increases according to the subject's performance. CCRT was conducted for each active group patient, 25-minutes sessions three times per week during 8 weeks.
    گروه آزمایش: مداخله شناختی این گروه از بیماران تحت تمرینات کامپیوتری بسته مغزینه توجه و تمرکز به منظور افزایش کارکردهای مغز قرار می‌گیرند. این بسته دارای 24 مرحله تمرینی است که به تدریج مشکل می‌شود. این مراحل در مدت 2 ماه (سه روز در هفته) توسط بیماران انجام می‌شود
    گروه آزمایش: مداخله شناختی. این گروه از بیماران علاوه بر بازتوانی روتین قلبی درمان بازتوانی شناختی کامپیوتری را به منظور فعال کردن عملکردهای مختلف شناختی شامل توجه، حافظه‌کاری و بازداری دریافت کردند. این بسته دارای 24 مرحله تمرینی است که به تدریج بر اساس عملکرد مراجع مشکل می‌شود.بازتوانی شناختی کامپیوتری برای هر بیمار، 25 دقیقه به مدت هشت هفته (سه روز در هفته) اجرا شد.

    Sham Group: This group Patients received a sham version of CCRT plus RCR. This package has 24 exercise steps.Each exercise doesn't depend on the subject's performance and its level of difficulty changes randomly. Sham version of CCRT was conducted for each sham group patient, 25-minutes session three times per week during 8 weeks.
    گروه پلاسیبو: این گروه از بیماران نسخه پلاسیبو تمرینات کامپیوتری بسته مغزینه را انجام می‌دهند. این بسته نیز شامل 24 مرحله تمرینی است و در طول دو ماه (3 بار در هفته) انجام می‌شود
    گروه پلاسیبو:این گروه از بیماران علاوه بر بازتوانی روتین قلبی نسخه پلاسیبو تمرینات شناختی کامپیوتری را دریافت کردند. این بسته نیز شامل 24 مرحله تمرینی است.مراحل تمرین به عملکرد آزمودنی بستگی نداشت و سطوح دشواری آن تصادفی تغییر می‌کرد. نسخه فیک بازتوانی شناختی کامپیوتری برای هر بیمار، 25 دقیقه به مدت هشت هفته (سه روز در هفته) اجرا شد.

Protocol summary

Study aim
The aim of this study was to assess the efficacy of CCRT as a supplementary treatment to RCR after CABG surgery in improving QoL. The outcome measures included both cognitive functioning and physical and mental components of QoL.
Design
The study was a randomized, single blind and parallel-group controlled trial with active, sham and control groups in which 75 outpatients were randomized by enrollment terms.
Settings and conduct
This study was carried out in Tehran Heart Center. The treatment protocol of patients was recorded and subjects completed the Quality of Life questionnaire (SF-36), then primary assessment of cognitive functions were done.
Participants/Inclusion and exclusion criteria
Inclusion Criteria: Male or female outpatients. Aged between 40 and 80. Capable of perception of the research nature. Able to give informed consent before any study is written. Capable to speak in Persian with study personnel. Able to read and write and work with computer. At least a month after CABG surgery. Participation in concurrent standard routine rehabilitation during the trial; Exclusion Criteria:Dependency on alcohol or drugs. Has any mental disorders that might interfere with the exact assessment or accuracy of treatment. Has any clinical conditions that cause the usage of prescribed drugs that could have any effect on the Attention.
Intervention groups
Patients in active group received CCRT plus RCR. CCRT was conducted for each active group patient, 25-minutes session three times per week during 8 weeks; Patients in the sham group received a sham version of CCRT plus RCR; Patients in the control group received RCR without any cognitive intervention for 2 months.
Main outcome variables
CCRT leads to significant improvement in the cognitive domains that have been trained; cognitive functions increase leads to significant improvement in the QoL in patients after CABG surgery.

Randomization description
This is a randomized controlled trial with pre-test, post-test and follow-up assessments in active, sham and control (no intervention) groups were conducted. 75 outpatients selected by convenience sampling among patients after CABG surgery. Randomization was carried out by the patients enrollment term in Tehran Heart Center(first term= active group, second term= sham group, third term= control group) .

The randomization of volunteer patients for participation in the study was carried out by the patients enrollment term in Tehran Heart Center (first term= active group, second term= sham group, third term= control group). clinical personnel were not aware of randomization according to enrollment day in the week. Scores of computerized training were recorded online and after the accomplishment of intervention, the data was received from related corporation.

Experimental Group:This group of patients received CCRT plus RCR to activate different cognitive functions including attention, working memory and inhibition. This package has 24 exercise steps that the difficulty of exercises gradually increases according to the subject's performance. CCRT was conducted for each active group patient, 25-minutes sessions three times per week during 8 weeks.
