The parallel randomized double-blind controlled clinical trial
a single-center, two-arm, parallel-group, double-blind randomized controlled phase 3 trial with a target sample size of 100 participants. Participants in Phase 3 will be randomly allocated in a 1:1 ratio using a computer-generated random sequence to LT4+SR-T3 combination therapy or LT4 monotherapy .
Setting: Tehran Thyroid Study (TTS). At the first visit, a fasting blood sample will be obtained to measure serum TSH, TT3, TT4, FT4, FT3, FBS, lipid profile, insulin, HbA1C, HOMA-IR, SHBG, Enolase, LDH and CK, Ferritin, and metabolomics. Assessment of demographic data, quality of life, symptoms of thyroid function, and physical examination will be done. Body composition will be measured. The specified treatment will be given to both groups for 48 weeks. All tablets will be distributed in similar packages and the same size and color to ensure double-blinded design. Participants will be evaluated at baseline and 3 follow-ups at first, six, and 12 months after the start-up. All blood parameters and questionnaires will be filled out at baseline and 6 and 12 months after start-up. Also, the polymorphism related to deiodinase will be measured and compared between responders and non-responders.
Tehran Thyroid Study (TTS). Participants will be randomly assigned to LT4 monotherapy or LT4 plus SR-T3 and will receive treatment for 48 weeks. The LT4 group will receive matching SR-T3 placebo to maintain double blinding. At baseline, 6 months, and 12 months, clinical assessments, quality of life, thyroid-related symptoms, body composition, resting energy expenditure, and biochemical and thyroid hormone measurements will be performed. Cardiovascular safety will be evaluated by resting heart rate, blood pressure, and ECG. Patient treatment preference will be assessed at 6 and 12 months. Pharmacogenetic analyses will evaluate rs225014, rs12885300, rs225015, and rs17606253.

Update 2: Reasons for the update The results of the clinical trial phase 2 were presented in a publication* and made us update the protocol of phase 3 in a few parts which are as follows: 1. We found that we should increase the ratio of SR-T3 to LT4 in the combination therapy. We also formulated SR-T3 in isolated tablets and not in one tablet/ capsule preparation due to the complexity of drug formulation and mixing a slow-release and normal-release product. In the new version, we consider treating hypothyroid patients with 75 µg lt4 and 15 µg sr-t3 and adjust the dosage based on the patient's weight. 2. We deleted the normal control group, there for our trial groups consist of one control group (LT4 monotherapy) and a trial group ( receiving 75 µg LT4 and 15 µg SR-T3) 3. We decided to measure all outcomes in 6 and 12 months after starting the trial instead of only at the end of the trial. 4. We also decided to measure body composition as another outcome of the trial. *Mehran L, Amouzegar A, Foroutan SM, Masoumi S, Tohidi M, Abdi H, Aghaei A, Saghafinia AE, Azizi F. Pharmacodynamic and pharmacokinetic properties of the combined preparation of levothyroxine plus sustained- release liothyronine; a randomized controlled clinical trial. BMC Endocr Disord. 2023 Aug 28;23(1):182. UPDATE 1 The phase 3 clinical trial had been approved previously by IRCT, but due to Covid-19 epidemic crisis, the trial was postponed. At the present time, we are ready to conduct the trial, however, based on the recently published Consensus Statements to guide development of best-designed future clinical trials of LT4+LT3 combination therapy by the American Thyroid Association (ATA), British Thyroid Association (BTA), and European Thyroid Association (ETA), we are going to make some changes in the protocol and update the previous trial in some parts including the extension of follow- up period up to 12 months, adding new outcomes (bone markers of C-telopeptide/N-telopeptide, Resting energy expenditure) and replacing the primary outcome to quality of life based on the suggestions made by the Consensus Statements1. Also, we added the complementary phase 2 clinical trial in a limited number of hypothyroid patients to evaluate the effectiveness and safety of monotherapy with our SR-T3 product. In the complementary phase 2 clinical trial we will select 30 patients with hypothyroidism (serum TSH>30 mU/L). These patients will be randomized into three groups receiving 1.6 μg/kg L-T4, equivalent doses of SRT3 and L-T3 of 0.55 μg/kg for 4 weeks and serum fT4, T3 and TSH will be measured weekly up to 4 weeks. This study is preliminary study to the previous approved clinical trial and the National Research Council of the Islamic Republic of Iran and the Human Research Review Committee of the Endocrine Research Center, Shahid Beheshti University, Tehran, Iran approved the complementary phase 2 protocol (IR.SBMU.ENDOCRINE.REC.1402.031). Trial participants will sign informed consent forms at baseline, and their personal information will remain strictly confidential. 1. UPDATE NO. 3 Subject: Request for amendment and harmonization of completed clinical trial record — IRCT20100922004794N12 Dear IRCT Administration Team, We respectfully request amendment and harmonization of the record for the clinical trial IRCT20100922004794N12, entitled “Formulation of Levothyroxine plus slow-release Liothyronine preparation for treatment of hypothyroidism (Clinical trial Phase 2 & 3).” The Phase 3 component of this trial has now been completed. During preparation of the final Phase 3 manuscript, we performed a detailed reconciliation of the current IRCT record with the final Phase 3 protocol and identified several residual inconsistencies originating from earlier versions of the registration. We wish to emphasize that all substantive Phase 3 methodological decisions described in this amendment were made before recruitment of the first Phase 3 participant and before any Phase 3 outcome data were available. The present request is therefore intended to harmonize the public registry with the final pre-recruitment Phase 3 protocol and actual trial conduct; it does not represent changes made in response to study results. Secondary outcomes Serum TSH, total T3, total T4, FT3, FT4 and the derived T3/T4, T3/FT4, and FT3/FT4 ratios were prespecified secondary biochemical outcomes. The proportion of participants with a T3/FT4 ratio within the predefined reference range of 65–145 was also a prespecified secondary biochemical analysis and should be added to the registered outcomes. Phase 3 treatment groups The final Phase 3 trial consisted of two randomized parallel treatment groups: 1. LT4 monotherapy. 2. LT4 plus slow-release liothyronine. The previously planned age- and sex-matched euthyroid control group was removed before Phase 3 recruitment. Although this deletion is already mentioned in the IRCT update history, the previous normal-control description remains in another section of the current registry. We respectfully request that this residual entry be removed or designated as superseded. Intervention and dose titration Participants allocated to combination treatment started therapy with LT4 plus SR-T3 at an initial SR-T3:LT4 dose ratio of approximately 1:5. The absolute starting doses were individualized according to each participant's thyroid hormone replacement requirement. The 1:5 ratio represented the initial treatment ratio and was not intended to remain fixed throughout follow-up. During the dose-adjustment period, LT4 and SR-T3 doses were individually titrated according to biochemical response, and consequently the SR-T3:LT4 dose ratio could change during follow-up. SR-T3 tablets containing 15, 20, and 25 μg were available for individual dose titration. LT4 tablets containing 50, 75, and 100 μg were also available. During the initial dose-adjustment phase, LT4 and SR-T3 doses were individually titrated according to biochemical response in order to maintain serum TSH within approximately 0.5–5.0 mU/L. In participants receiving combination treatment, the serum T3/FT4 ratio and the intended balance between LT4 and SR-T3 exposure were also considered during titration. We therefore request that the intervention section be revised to describe the starting regimen and subsequent individual titration rather than implying that the 1:5 ratio remained fixed throughout the study. LT4 monotherapy group Participants assigned to the control group received individualized LT4 replacement according to their pretrial replacement requirement and biochemical response, with dose adjustment when required to maintain euthyroidism. The control arm was not restricted to a fixed dose of 100 μg LT4 throughout follow-up. We therefore request correction of the registry section that currently describes a fixed 100-μg/day LT4 regimen. Placebo and masking Participants randomized to LT4 monotherapy received a matching placebo corresponding to SR-T3 in order to maintain double blinding. Active SR-T3 and placebo tablets were matched in physical appearance, taste, smell, and packaging. The current IRCT record contains an internal inconsistency in which the masking description correctly states that the control group received placebo instead of SR-T3, whereas another field states “Placebo: Not used.” We therefore request that the placebo field be corrected to indicate placebo used. Eligibility criteria The Phase 3 study included adults aged 20–75 years with hypothyroidism who were euthyroid on stable LT4 treatment before randomization. The final pre-recruitment exclusion criteria also included pregnancy or planned pregnancy and clinically important cardiovascular conditions, including unstable cardiovascular disease and cardiac arrhythmia. We additionally request clarification of the cancer-related eligibility wording so that the registry clearly distinguishes previous definitively treated thyroid cancer resulting in hypothyroidism from current or active malignancy, according to the final Phase 3 protocol. Follow-up period The Phase 3 intervention and follow-up period was 48 weeks, approximately 12 months, with principal assessments at baseline, 6 months, and 12 months and additional biochemical monitoring during the initial dose-adjustment period. We request correction of the isolated registry reference to “24 weeks,” which is inconsistent with the final Phase 3 protocol. Cardiovascular safety assessments Prespecified cardiovascular safety monitoring included resting heart rate, blood pressure, and serial electrocardiography. ECG variables included heart rate, RR interval, PR interval, QRS duration, QT interval, corrected QT interval, and clinically important rhythm and conduction abnormalities. ECG assessments were performed at baseline, 6 months, and 12 months. We request that these ECG assessments be added to the registered safety procedures and outcomes. Pharmacogenetic assessments The Phase 3 Pharmacogenetic analyses evaluated the candidate variants: rs225014, rs12885300, rs225015, and rs17606253. We request replacement of the current broad genetic description and reference to whole-genome sequencing with the actual candidate-variant analysis and laboratory methodology used in the study. The final laboratory method will be entered once all laboratory details have been confirmed: Patient preference Patient treatment preference was assessed during follow-up and at study completion. We request correction of the registered assessment timing where necessary. We therefore request completion of the following fields: Actual recruitment start date: 2024.12.03 Actual recruitment end date: 2024.12.08 Final participant/final study visit date: 2025.12.10 Post-trial treatment Following completion of assigned study treatment, participants were returned to conventional LT4 replacement therapy and followed until biochemical euthyroidism was re-established. We respectfully request that the above amendments be incorporated while preserving the original registration history and dates of previous protocol updates. The purpose of this amendment is to provide a complete, accurate, and transparent public record of the final Phase 3 protocol and actual study conduct. We again confirm that the substantive Phase 3 methodological decisions described above were established before Phase 3 recruitment began and before any Phase 3 outcome data were available. Please let us know if supporting documents, including dated protocol versions, ethics approvals, investigational-product documentation, or study records, are required.
Pharmacodynamic and pharmacokinetic properties of the combined preparation of levothyroxine plus sustained-release liothyronine; a randomized controlled clinical trial. BMC Endocr Disord. SBMU.ENDOCRINE.REC.1402.031) را تایید کردند. Phase 3: Patients will be allocated to intervention and control groups using simple stratified randomization. 1. The groups with a daily intake of LT4+SR-T3 (Intervention Group) 2. The groups with LT4 mono-therapy (Control group) Participants will be randomized with equal probability (1:1) to receive one of the two treatments. As the size of each group is predicted to be 50 , the allocation sequence is generated with sample randomization and stratification by gender. The sequences will be generated by the software and in Excel format. The patients sequentially entered to study based on this random sequence. Phase 2: After simple randomization, participants will be assigned to a pre-breakfast regimen of SRT3, L-T3, or L-T4 for 4 weeks. The dose of L-T4 is 1.6 μg/kg. This dosage will be adjusted to achieve 1:5 ratio of SR-T3 to LT4 in combination therapy. Patients will be recalled to the endocrine clinic for weekly follow-up visits and blood sampling.
Phase 3: Participants will be randomly allocated in a 1:1 ratio to one of two parallel treatment groups: LT4 plus slow-release liothyronine (SR-T3) combination therapy (intervention group). LT4 monotherapy (control group). A computer-generated random allocation sequence will be prepared to assign participants to the two treatment groups with equal probability. The allocation sequence will be kept confidential, and participants will be enrolled sequentially according to the generated randomization sequence. Phase 2: After simple randomization, participants will be assigned to one of three pre-breakfast treatment regimens: SR-T3, L-T3, or L-T4, for 4 weeks. The L-T4 dose will be 1.6 μg/kg, and the corresponding SR-T3 and L-T3 doses will be administered according to the Phase 2 protocol. Participants will attend the endocrine clinic for weekly follow-up visits and blood sampling during the 4-week treatment period.
After proper implementation of randomization, the subjects will be assigned to the groups using allocation concealment which helps to keep clinicians, participants and investigators unaware of upcoming assignments. The standard methods of ensuring allocation concealment will be sequentially numbered or coded opaque containers. For single-center clinical trials such as the current trial, we will identify a staff member not involved with the trial who can keep the randomization list. This staff will be instructed to keep the list private and to only reveal a treatment allocation after receiving information demonstrating that the patient is eligible and has consented to the trial. Both the subjects and the investigators will be kept from knowing who will be assigned to which treatment (double-blind) to fulfill this both groups will receive tablets that are identical in physical appearance, taste, and smell. The patients in the control group will receive Placebo instead of SR-T3.
The random allocation sequence will be concealed from investigators and clinical staff involved in participant recruitment and assessment. The allocation sequence will be kept confidential by a designated staff member who will not be involved in participant recruitment, clinical management, or outcome assessment. After eligibility has been confirmed and written informed consent has been obtained, the participant will be assigned the next allocation code according to the randomization sequence. The treatment will be provided using the corresponding coded study medication without revealing the assigned treatment to participants or investigators. Both participants and investigators will remain blinded to treatment allocation. Participants assigned to the LT4 monotherapy group will receive a matching placebo corresponding to SR-T3. Active SR-T3 and placebo tablets will be matched in physical appearance, taste, smell, and packaging to maintain double blinding.

Tavan Institute and Dorsa Pharmaceutical Company will formulate SR-T3 tablets (15 µg). Iran Hormone Company produces 50, 75, and 100 µg levothyroxine tablets. The patients will be randomly assigned to intervention (with a daily intake of LT4+SR-T3; 1:5 ratio) and control groups. The tablets should be taken in the morning at least half an hour before breakfast. At the first visit, a fasting blood sample will be obtained at 8 am and the specified treatment will be started on the same day. Participants will be evaluated at baseline and 3 consequent follow-ups ( third, sixth, and 12 months after the start-up trial) up to 48 weeks. At each follow-up visit, venous blood samples will be collected from all participants after a 12-hour fast to measure serum TSH, total T3, total T4, free T4, and free T3. At the first and last visit total cholesterol, LDL, HDL, triglycerides, FBS, insulin, HbA1C, HOMA-IR, SHBG, Enolase, LDH and CK, Ferritin, C-telopeptide, N- N-telopeptide and metabolomics will be measured and heart rate, blood pressure will be evaluated. All questionnaires (Thyroid symptoms, Thypro-39 QOL) will be filled out at the first and last visits(6 and 12 months of start-up). The drug will be continued for 48 weeks. Patients will also be checked at 4 weeks intervals to measure TSH, assess adherence to therapy and adverse effects. Drug dosage would be adjusted to maintain serum TSH concentration within 0.5-5 mU/l.
Participants randomized to combination therapy will start treatment with LT4 plus slow-release liothyronine (SR-T3) at an initial SR-T3:LT4 dose ratio of approximately 1:5. The absolute starting doses of LT4 and SR-T3 will be individualized according to each participant's thyroid hormone replacement requirement. The 1:5 ratio will represent the initial treatment ratio only and will not be required to remain fixed throughout follow-up. During the dose-adjustment period, LT4 and SR-T3 doses will be individually titrated according to biochemical response in order to maintain serum TSH within approximately 0.5–5.0 mU/L. In participants receiving combination therapy, the serum T3/FT4 ratio and the intended balance between LT4 and SR-T3 exposure will also be considered during dose titration. Consequently, the SR-T3:LT4 dose ratio may change during follow-up.SR-T3 tablets containing 15, 20, and 25 μg and LT4 tablets containing 50, 75, and 100 μg will be available for individualized dose adjustment. Study treatment will be administered once daily in the morning before breakfast and will continue for 48 weeks. After completion of study treatment, participants will return to conventional LT4 replacement and will be followed until biochemical euthyroidism is re-established.

General information

Reason for update

Randomization description
Blinding (investigator's opinion)
Double blinded
Blinding description
